Incidence análních premaligních lézí a karcinomu anu v poslední době stoupá, a to zejména mezi rizikovými skupinami populace. V současné době v České republice neexistuje žádný preventivní screeningový program, ale do budoucna se předpokládá jeho zavedení alespoň mezi rizikové skupiny populace. Teoretická část bakalářské práce seznamuje čtenáře s obecnými informacemi o lidských papilomavirech, ale i o způsobu jejich detekce a možné prevenci. V experimentální části byly na výsledcích vzorků od 150 HIV pozitivních mužů majících sex s muži porovnány dvě metody, které by mohly být pro screening využity. Jako vhodnější se ukázala metoda Anyplex II HPV 28 před metodou GP5+/6+ RLB. Výhodou metody Anyplex II HPV 28 je možná automatizace testu a fakt, že je vhodnější pro detekci vzorků s multiplicitními infekcemi HPV než metoda GP5+/6+ RLB. Obě tyto metody by však bylo vhodné používat pro triáž pacientů v uvažovaném screeningovém programu karcinomu anu. Dále bylo zjištěno, že samotná detekce HPV není dostačující pro přesné určení diagnózy. Do uvažovaného screeningu bude tedy pravděpodobně nutné zařadit molekulárně biologickou detekci HPV spolu s cytologickým vyšetřením. Bakalářská práce také potvrdila nutnost více propagovat primární prevenci, tedy očkování. V případě proočkovanosti sledované rizikové skupiny vakcínami dostupnými na českém trhu by se až 36 % sledovaných pacientů nyní nepotýkalo s infekcí vysoce rizikovými HPV typy. Recently the incidence rate of anal intraepithelial neoplasia and anal carcinoma has been increasing mainly in the risk population. Even though no screening programme used for prevention is currently available in the Czech Republic, its implementation into the risk populations is expected in the future. The theoretical part of the bachelor thesis presents general information about Human papillomaviruses as well as methods of their detection and possibilities of prevention. The results of the two perspective screening methods from 150 HIV positive men having sex with men were compared in the experimental part of the thesis. The Anyplex II HPV 28 method has proved to be more eligible than the GP5+/6+ RLB testing considering the possibility of automation and capability of detection of multiple HPV infections. However, both methods are adequate for triage in the screening programme for anal cancer. Furthermore, it has been found out that HPV detection alone is not sufficient for an accurate diagnosis. Therefore, it will be necessary to include another molecular biological detection method of HPV together with a cytological examination of risk patients in the screening. The bachelor thesis also has confirmed the necessity to promote primary prevention, especially vaccination. It has been concluded that up to 36 % of subjects from the monitored risk group of patients would not have suffered from high-risk HPV type infection if they have been vaccinated with prophylactic vaccines available in the Czech market.
