Telefonicky asistovaná neodkladná resuscitace u dospělé osoby

Abstract

Bakalářská práce se zabývá problematikou telefonicky asistované neodkladné resuscitace u dospělé osoby. Cílem bylo zjistit zhodnocení reálných situací při příjezdu výjezdových skupin na místo události, kde probíhá TANR a také zjistit, zda je podle operátorů TANR přínosná. Stanovené byly čtyři hypotézy. V teoretické části je stručně popsána zdravotnická záchranná služba a zdravotnické operační středisko. Součástí teoretické části je náhlá zástava oběhu, její příčiny, klinický obraz a prevence. Také jsou zde obsaženy základní informace o řetězci přežití. A v závěru teoretické části je popsána telefonicky asistovaná neodkladná resuscitace, její indikace, kontraindikace, postup TANR u dospělé osoby, dále její ukončení a s ní související studie DIRECT. Vyhodnocení výsledků, které je součástí praktické části práce, probíhalo na základě dvou anonymních dotazníků. Jeden z dotazníků byl určen pro členy výjezdových skupin zdravotnické záchranné služby, tj. zdravotnické záchranáře, lékaře a řidiče. Druhý dotazník byl určen pro operátory zdravotnického operačního střediska. Námi získané výsledky ukazují, že na místě události, kde probíhá telefonicky asistovaná neodkladná resuscitace, se zachránce většinou řídí pokyny operátora, ale přesto není resuscitace poskytována správně. Podle operátorů je TANR jednoznačně přínosná. Dvě ze stanovených hypotéz byly potvrzeny, naopak dvě vyvráceny.

This bachelor thesis deals with the matters of Telephone-assisted Cardiopulmonary Resuscitation in adult. The goal was to find the evalutation of real-life situations during the arrival of emergency services on the incident site, where TACR is occuring and to find out, whether or not TACR is beneficial. Four hypotheses were set. In the theoretical part, the emergency medical service and the medical operation facility is briefly described. Part of the theory is also the sudden cardiac arrest and it's causes, clinical image and prevention. Basic information about the chain of survival is included as well. Towards the ending, TACR is discribed as well as it's indications, contradictions, process and finishing and the DIRECT study. The evaluation of the results, which is part of the practical part, was made thanks to two anonymous questionnaires. One of them was directed towards emergency responders, doctors and drivers. The second was made for the emergency phone operators. The results we aquired show, that on the incident site, although the rescuer is usually following the directions of the operator, the resuscitation is still not done properly. The operators say, that TACR is definitely beneficial. Two of the set hypotheses were correct, the other two were not.
