Mozkový kmen a mozeček jsou velmi složité části lidského těla obsahující velké množství nervových jader drah ovládajících různé části lidského těla od motoriky, přes čití, po respiraci a krevní oběh. Tyto struktury jsou však natolik malé, že je nejsme ani s pomocí moderních zobrazovacích metod přesně identifikovat. Cílem této práce bylo tedy najít vhodnou metodu pro identifikaci těchto struktur na snímcích z magnetické rezonance (MR). Pro určení pozicí těchto struktur byla zvolena metoda registrace obrazových dat. Registrován byl schématický snímek z topografického atlasu mozkového kmene a mozečku za pomoci po částech lineární afinní transformace. Registrovaný snímek byl poté proložen se snímkem z magnetické rezonance. Na MR snímcích byly dále identifikovány léze, které byly zobrazeny na proložený snímek. Byl vypočten překryv oblastí jednotlivých struktur lézemi. Metoda byla otestována na dvanácti pacientech trpících roztroušenou sklerózou se specifickými symptomy, které vypovídají o zasažení známých struktur. Dále bylo využití metody porovnáno na třech lékařích. Jeden neurolog a dva lékaři s jinou specializací. Byly srovnávány parametry vybírání lézí a to velikost léze a kruhovitost. U dvanácti pacientů bylo určeno zasažení naprosté většiny struktur podle předpokladu. Z testování použitelnosti lékaři jsme dospěli k závěru, že nejvhodnější uživatel je neurolog. Lékaři bez neurologické specializace měli tendenci zkreslovat výsledky a to jako falešně negativní. Celkově byla metoda ohodnocena jako použitelná v klinické praxi pro identifikaci drobných struktur mozkového kmene a mozečku. Bylo by však vhodné metodu rozšířit o více struktur, než které byly využity v rámci pilotní studie. Brainstem and cerebellum are very complex parts of the human body containing great number of nerve units and runways, which control different functions of the body ranging from the motor function and the sensory function to respiration or circulation. These structures are so miniscule, that even with help of modern imaging methods, we are not able to identify their exact position. Main goal of this thesis is to find a method to identify their positions on magnetic resonance (MR) images. A method of image registration was chosen. A schematic image from the topographic atlas of brain stem and cerebellum was registered on the MR images using the piece-wise linear affine transformation. The registered image was the fused with the MR image. Lesions on the MR image were circumscribed and displayed on fused image. Overlaps of the lesion on structures were calculated. The method was tested on 12 patients diagnosed with multiple sclerosis and with specific syndromes, which were caused by the damage to known structure of brainstem of cerebellum. The method was also compared between 3 medical physicians. One neurologist and two physicians with different specialization. Lesion parameters as size and roundness were compared for every physician specifically. The vast majority of predicted damaged structures was identified as damaged by this method. By testing between physicians we concluded, that the best user is, as predicted, a neurologist. Physicians without neurological specialization were biased to identify smaller sized lesions and so creating the false negative results. Whole method was rated as usable in clinical practice for identification of small structures in brainstem. The next step would be to expand the library of the structures to have more then what was used for pilot study.
