Návrh modelu nákupu infuzní a enterální techniky z hlediska technických parametrů a nákladové analýzy

Abstract

Diplomová práce měla několik cílů: zmapovat současný stav studií využívaných pro nákup lékařské techniky, zmapovat současný trend nákupu infuzní a enterální techniky na anesteziologicko-resuscitačních odděleních v České republice a pomocí vybraných metod HTA stanovit nejefektivnější model nákupu vybrané lékařské techniky. V rámci současného stavu problematiky byly analyzovány studie týkající se nákupu lékařských přístrojů. Z těchto studií byly vybrány nejvhodnější metody, které byly aplikovány v praktické části práce. Následně byl proveden průzkum na anesteziologicko-resuscitačních odděleních s cílem zjistit současný stav využívaných infuzních a enterálních pump. Data z dotazníků byla vyhodnocena pomocí Paretova diagramu a byly vybrány nejčastěji zastoupené typy infuzní a enterální techniky. Následně byla pomocí vybraných odborníků stanovena kritéria přístrojů a stanoveny váhy těchto kritérií. Pomocí vybraných metod TOPSIS a AHP byly vypočteny efekty pro jednotlivé infuzní a enterální pumpy. Následně byla vypočtena nákladová analýza pro všechny vybrané varianty, které byly mezi sebou porovnány. Nejvyšší nákladovou efektivitu vykázal za infuzní i za enterální pumpu přístroj Infusomat Space od firmy B. Braun Medical s.r.o. Z výsledků je patrné, že i přes vyšší pořizovací cenu je tento model pro toto oddělení z hlediska požadovaných kritérií nejefektivnější.

This Master?s thesis had several goals: to map the current state of studies used for buying medical devices, to map current buying trend of infusion and enteral techniques for anesthesiology and resuscitation department in the Czech Republic, and with a help of selected methods HTA determine the most effective buying model for selected medical devices. In regards to the current problem only studies focused on buying medical device were analysed. From these studies, the most appropriate methods were chosen and were applied in the practical part of this thesis. Subsequently, a research of anesthesiology and resuscitation department was conducted with the attempt to determine the current state of used infusion and enteral pumps. Data from the questionnaires were processed by Paret?s diagram and the most frequently used types of infusion and enteral devices were selected. Consequently, criteria of the medical tools and its weight were determined by selected professionals. The effect of individual infusion and enteral pumps were calculated through selected TOPSIS and AHP methods. After, an expense analysis was calculated for all selected variations that were compared. In regards to cost effectiveness, Infusomat Space produced by B. Braun Medical s.r.o. had the highest cost-effectiveness ratio. The analysis revealed that despite higher purchase cost this model is the most cost effective for this department.
