Analýza nákladové efektivity kochleárních implantátů u dospělých

Abstract

Diplomová práce se zabývá problematikou kochleárních implantátů. Zahrnuje stručný teoretický úvod do problematiky ztráty sluchu a sluchových vad. Dále jsou popsány naslouchadla a právě kochleární implantáty. Tyto pomůcky mají přínos nejen v možnosti výměny informací, ale zlepšují i psychický stav pacientů a umožňují jim zůstat profesně aktivní. Současný stav v České republice, zahrnuje nejen zmínku o možných místech pro implantaci, ale i situaci s proplácením sluchových pomůcek. Kapitola současného stavu v zahraničí rozebírá studie z jednotlivých států a popisuje jejich výsledky. Dále navazuje shrnutí všech studií zkoumaných v rámci této práce. Analýza nákladové efektivity byla provedena pomocí Markovových modelů. Náklady do analýzy byly brány z perspektivy plátce, a to ze Seznamu zdravotních výkonů a výsledná cena byla vypočtena dle Úhradové vyhlášky. Utility a pravděpodobnosti vzniku jednotlivých stavů byly převzaty ze studií. Výsledky analýzy ukazují, že kochleární implantáty ve srovnání s naslouchadly u dospělých pacientů jsou nákladově efektivní. ICER vyšel 259 806 Kč za jedno QALY. Následná probabilistická analýza senzitivity prokázala, že kochleární implantát je nákladově efektivní intervencí. Z celkového počtu 10 000 simulací bylo více jak 50 % nákladově efektivních. Byla vytvořena i analýza scénářů. Všechny čtyři alternativní scénáře taktéž prokázaly nákladovou efektivnost kochleárních implantátů. Závěrem je diskutován současný stav s kochleárními implantáty v České republice a ve světě. Výsledky diplomové práce jsou porovnány s výsledky zahraničních studií.

The diploma thesis deals with the issue of cochlear implants. It includes a brief theoretical introduction to the problem of hearing loss and auditory disorders. Furthermore, it describes hearing aids and specifically cochlear implants. These devices have benefits not only in terms of information exchange, but also improve the psychological well-being of patients and enable them to remain professionally active. The current situation in the Czech Republic includes not only mention of possible places for implantation, but also the situation with reimbursement of hearing aids and cochlear implants. The chapter on the current situation abroad analyzes studies from various countries and describes their results. It is followed by a summary of all the studies examined in this work. Cost-effectiveness analysis was performed using Markov models. The costs for the analysis were taken from the payer’s perspective, specifically the List of Healthcare Procedures, and the resulting price was calculated according to the Reimbursement Decree. Utilities and probabilities of occurrence of individual states were adopted from studies. The results of the analysis show that cochlear implants are cost-effective compared to hearing aids in adult patients. The ICER amounted to 259,806 CZK per QALY. Subsequent probabilistic sensitivity analysis demonstrated that the cochlear implants are a cost-effective intervention. Out of a total of 10,000 simulations, more than 50 % were cost-effective. A scenario analysis was also conducted, and all four alternative scenarios also demonstrated the cost-effectiveness of cochlear implants. In conclusion, the current situation of cochlear implants in the Czech Republic and in the world is discussed. The results of the diploma thesis are compared with the results of foreign studies.
