Využití základních hematologických parametrů k odhadu klinického průběhu COVID 19

Abstract

Cílem předkládané diplomové práce bylo nalézt vztah mezi hematologickými laboratorními parametry a parametry klinického průběhu onemocnění COVID-19. Dílčím cílem diplomové práce bylo statistické vyhodnocení možných vztahů nejdůležitějších hematologických parametrů a tohoto onemocnění. V teoretické části souhrnně popisuji klinický průběh a stav onemocnění způsobené virem SARS-CoV-2 včetně jeho léčby, přehled současné epidemiologické situace ve světě. Literární přehled je rozšířen také o náhled na tuto problematiku v zahraničních i českých studiích zabývajících se především dopadem působení viru na hematologické parametry a metabolismus pacienta. Metodická část práce se zabývá zpracováním získaných klinických dat z databáze informačního systému infekční nemoci (ISIN) a hematologických dat z laboratorního informačního systému (LIS) v Ústřední vojenské nemocnici-Vojenské fakultní nemocnici Praha (ÚVN-VFN Praha) a následným statistickým zpracováním souboru dat pomocí statistických testů (Mann-Whitneyho test, Kruskal-Wallisova ANOVA či logistická regrese). Získané výsledky ukazují, že se hematologické parametry liší dle vybraných skupin charakterizujících průběh onemocnění (UPV, ECMO, HFNO, kyslík, mortalita, JIP), také dle pohlaví. Vždy se liší minimálně v jednom parametru diferenciálního rozpočtu leukocytů. Byl nalezen vztah mezi hematologickými parametry (Baso S a IG %), pohlavím, věkem pacienta a mortalitou. Také byly nalezeny rozdíly hematologických parametrů dle závažnosti stavu pacienta.

The aim of the present thesis was to find the relationship between haematological laboratory parameters and the parameters of the clinical course of COVID-19. A sub- bjective of the thesis was to statistically evaluate possible relationships between the most important haematological parameters and this disease. In the theoretical part, I summarise the clinical course and status of the disease caused by SARS-CoV-2 virus including its treatment, review the current epidemiological situation in the world. The literature review is also expanded by the insight into this issue in foreign and Czech studies dealing mainly with the impact of the virus on the haematological parameters and metabolism of the patient. The methodological part of the thesis deals with the processing of clinical data obtained from the Infectious Disease Information System (ISIN) database and haematological data from the Laboratory Information System (LIS) at the Central Military Hospital –The Military University Hospital Prague (ÚVN-VFN Prague) and the subsequent statistical processing of the sets of data using the statistical tests (Mann-Whitney test, Kruskal-Wallis ANOVA or logistic regression). The obtained results show that the haematological parameters differ according to selected groups characterizing the course of the disease (UPV, ECMO, HFNO, oxygen, mortality, ICU), also according to gender. They always differ in at least one parameter of the differential leukocyte budget. A correlation has been found between the haematological parameters (Baso S and IG %), gender, patient age and mortality. The differences in the haematological parameters according to the severity of the patient's condition were also found.
