Pacientské preference u léčby osteoartrózy kloubů dolních končetin

Abstract

Diplomová práce je věnována problematice pacientských preferencí u léčby osteoartrózy kloubů dolních končetin v České republice. Jedná se o degenerativní nezánětlivé onemocnění, které postihuje až 15 % celkové populace nad 55 let věku. Jelikož neexistuje žádná účinná léčba kromě výměny postiženého kloubu, pacienti s tímto onemocněním mohou mít dilema, který léčebný přístup zvolit. Byla provedena literární rešerše v zahraničí, jelikož v České republice nebyly na toto téma identifikovány žádné relevantní studie ani články. Ze zahraniční rešerše vyplývá, že je důležité měřit pacientské preference, jelikož preference pacienta nemusí vždy korelovat s tím, co je klinicky nejlepším řešením. Cílem této diplomové práce bylo zanalyzovat pacientské preference u léčby osteoartrózy kloubů dolních končetin a zjistit vzájemné důležitosti mezi jednotlivými atributy léčby tohoto onemocnění. V práci byla použita metoda Discrete Choice Experiment, která byla aplikována pomocí dotazníkového šetření v 6 vybraných zdravotnických centrech v České republice. Výzkumu se zúčastnilo 110 respondentů, kteří odpovídali na 6 otázek, ve kterých vybírali pro ně preferovanou variantu léčby. Ta byla popsána 5 atributy, které byly stanoveny na základě zahraničních studií a konzultací s odborníky: intervence, riziko vzniku mírných komplikací, riziko vzniku závažných komplikací, bolestivost a pohyblivost postiženého kloubu. Výsledky byly analyzovány v programu R a vyplývá z nich, že pro pacienty s osteoartrózou v České republice je nejdůležitějším atributem léčby riziko vzniku mírných komplikací následované bolestivostí. Naopak nejméně důležitým atributem je s velkým odstupem pohyblivost postiženého kloubu. Byly provedeny analýzy i jednotlivých podskupin respondentů, ze kterých vyplývá, že pro různé podskupiny existují rozdílné preference.

This diploma thesis is devoted to the issue of patient preferences in the osteoarthritis treatment of the lower limbs’ joints in the Czech Republic. It is a degenerative non-inflammatory disease that affects up to 15 % of the total population over 55 years of age. Since no effective treatment other than replacing the affected joint exists, the patients with this condition may be in a dilemma as to which treatment approach to choose. A foreign literature research was conducted, as no relevant studies or articles from the Czech Republic on this topic were identified. The international research shows the importance of measuring patient preferences, as patient preferences may not always correlate with what is the best clinical solution. The aim of this diploma thesis was to analyze patient preferences in the osteoarthritis treatment of the lower limbs’ joints and to determine the mutual importance between the individual attributes of the treatment of this disease. The Discrete Choice Experiment method was used in this research, which was applied using a questionnaire survey in 6 selected health care centers in the Czech Republic. The research was participated in by 110 respondents who answered 6 questions in which they chose their preferred treatment option. It was described by 5 attributes that were determined based on foreign studies and consultations with experts: intervention, risk of mild complications, risk of serious complications, pain and mobility of the affected joint. The results were analyzed in the R program, and they show that for patients with osteoarthritis in the Czech Republic, the most important attribute of treatment is the risk of mild complications followed by pain. On the contrary, the least important attribute is the mobility of the affected joint by a large margin. Analyzes were also carried out for individual subgroups of respondents, which show that there are different preferences for different subgroups.
