Tato diplomová práce se zabývá problematikou diabetické retinopatie, screeningem a léčbou v souvislosti s nákladovou efektivitou z pohledu pojišťovny a využitím QALY jako efektu terapie. Cílem práce bylo vytvořit model progrese onemocnění pro hodnocení screeningového programu. Pro vytvoření tohoto modelu byla použita metoda diskrétních simulací (simulace událostí), dále byly využity metody HTA a analýzy citlivosti. Dle výsledků simulací vychází screening jako dominantní intervence, kdy poskytuje více efektu za menší náklady, avšak citlivostní analýzy, zvláště probabilistická citlivostní analýza, ukázaly heterogenitu výsledků. This diploma thesis deals with the issue of diabetic retinopathy, screening, and treatment in connection with cost-effectiveness from the perspective of the insurance company and the use of QALYs as an effect of therapy. The aim of the work was to create a model of disease progression for the evaluation of a screening program. The method of discrete simulations (event simulation) was used to create this model, as well as HTA methods and sensitivity analyzes. According to the results of the simulations, screening is based on a dominant intervention, providing more effect at lower cost, but sensitivity analyzes, especially probabilistic sensitivity analysis, have shown heterogeneity of results.
