Analýza nákladové efektivity u léčby ženské neplodnosti pomocí in vitro fertilizace a intrauterinní inseminace

Abstract

Analýza nákladové efektivity u léčby ženské neplodnosti pomocí IVF a IUI Ženská neplodnost je onemocnění, které postihuje celosvětově stále více žen. S tím souvisí zvyšující se zájem o léčbu metodami asistované reprodukce. Ekonomická náročnost těchto metod má rostoucí tendenci. Diplomová práce analyzujte současný stav problematiky IVF a IUI v ČR a ve světě. Hlavním cílem je vyhodnocení nákladové efektivity metod IVF a IUI v ČR. Práce se zabývá vyhodnocením nákladů vynaložených na IVF a IUI a následně nákladovou efektivitou těchto dvou metod. K vyjádření nákladové efektivity je využita metoda CEA - cost effectiveness analysis a ICER – Incremental cost effectiveness ratio. Nákladová efektivita je posuzována z pohledu plátce. Tím je v tomto případě zdravotní pojišťovna se spoluúčastí samotné pacientky. Při výpočtu nákladů bylo zjištěno, že náklady na IUI činí téměř 9 000 Kč a náklady na IVF se pohybují mezi 70 000 – 90 000. Výše nákladů se liší v závislosti na variabilitě několika faktorů. Pro účely práce je sestavena modelová situace za použití Rozhodovacího stromu. Nákladová efektivita je hodnocena pro tři způsoby léčby a to IUI, IVF a kombinaci těchto dvou metod. V diplomové práci bylo zjištěno, že kombinace IUI a IVF má nejen nejvyšší efekt co se týká úspěšnosti, a to 0,9576 (95,76 %), ale je také nákladově nejefektivnější. Cílem práce je nejen určit, která z metod je nákladově efektivnější, ale porovnat negativa a pozitiva obou metod a zvážit zvýšení věkové hranice pro úhradu cyklů IVF ze zdrojů veřejného zdravotního pojištění. Úspěšnost léčby s věkem významně klesá a rostou náklady na léčbu. V případě zvýšení věkové hranice pro úhradu cyklů IVF ze zdrojů veřejného zdravotního pojištění, které legislativa zvažuje, by měl být na tento fakt brán zřetel.

Cost-effectiveness analysis of IVF and IUI treatment of female infertility Female sterility is a disease which affects more ande more women worl-wide. It´s connected with an increasing interest in the treatment of assisted reproduction methods. The economic demands of these methods have a growing tendency. This Thesis analyzes present state of the problematics of IVF and IUI in the Czech Republic and in the world. Although the main aim is to evaluate the cost-effectiveness of the methods IVF and IUI in the Czech Republic. Methods like CEA - cost effectiveness analysis a ICER – Incremental cost effectiveness ratio are used to comments on cost of effectiveness. Cost effectiveness is assessed from the payer´s viewpoint. In this case it´s the health insurance company with the participation of the patient herself. Following the calculation of costs was found that the costs of IUI are almost 9000 Czech crowns and the costs of IVF are around 70 000 - 90 000 Czech crowns. The amount of costs varies depending on the variability of several factors. For the purposes of this Thesis a model situation is drawn up by using a decision tree. Cost-effectiveness is evaluated for three treatments. These three treatments include IUF, IVF and a combination of the methods above. The study found that the combination of IUI and IVF have the highest effect in term of success at 0,9576 (95,76 percent), but it´s also the most cost-effective. The aim of the work is not only to determine which of the methods is more cost-effective, but also compare the negatives and positives of both methods and make a decision to raising the age limit for the payment of IVF cycles from public health insurance sources. The success of treatment decreases significantly with age and the cost of treatment increases. In the case of raising the age limit for the payment of IVF cycles from public health insurance sources, which is considered by the legislation - this fact should be taken.
