Problematika CT periradikulární terapie a facetové denervace se zaměřením na edukaci pacientů

Abstract

Tato bakalářská práce se zabývá problematikou CT periradikulární terapie a facetové denervace se zaměřením na edukaci pacientů, jakožto informačním procesem nezbytným při poskytování kvalitní zdravotní péče a úlohou radiologického asistenta v tomto procesu. V teoretické části je popsán proces edukace, anatomie nervové soustavy, rozsáhleji centrální nervová soustava a stručněji periferní nervová soustava a základní anatomie axiálního pohybového aparátu. Dále pojednáváme o patofyziologických indikacích k periradikulární terapii a facetové denervaci. V kapitole 3.4 popisujeme jmenované výkony, přípravu pacienta a vyšetřovny CT, postup při výkonech a péči o pacienta po výkonech, očekávané přínosy výkonů, a také zmiňujeme související komplikace a kontraindikace. V kapitole 3.5 uvádíme zobrazovací metody spojené s intervenčními výkony, konkrétně výpočetní tomografii a s diagnostikou patofyziologických indikací - magnetickou rezonanci. Konečně zmiňujeme úlohu radiologického asistenta v radiační ochraně pacientů a pracovníků. V praktické části jsme získali data pro kvantitativní výzkum pomocí strukturovaného dotazníku. Touto cestou jsme zjišťovali míru edukace pacientů před výkonem. Výsledky jsme uvedli s doprovodem grafického znázornění pomocí sloupcových grafů. Následně jsme vyhodnotili stanovené hypotézy - hypotézy č. 1, 2, 4, 5, 9 jsme přijali a č. 3, 5, 7, 8 jsme zamítli. Na základě výsledků dotazníkového šetření, zejména otázek č. 10 a 12, jsme zhotovili přehledný a jednoduchý informační leták, který má pomoci při edukaci pacientů, nicméně je rovněž určen pro širokou veřejnost. Informační leták jsme distribuovali na pracoviště radiodiagnostického oddělení Ústřední vojenské nemocnice - Vojenské fakultní nemocnice Praha a Nemocnice Na Homolce, jeho cílem je zvýšení míry edukace a informovanosti, zlepšení spolupráce pacientů při výkonech a snížení obav před tímto zákrokem.

This bachelor thesis deal with the issue of CT periradicular therapy and facet-joints denervation with a focus on patient education as an information process necessary in providing quality health care and the role of the radiological assistant in this process. The theoretical part gives a description of education process, anathomy of nervous system, a more extensive description of central nervous system and less extensive description peripheral nervous system and basic anathomy of axial locomotor system. Also, we deal with patophysiological indications for periradicular therapy and facet-joints denervation. In Chapter 3.4 we desribe the mentioned interventions, patient preparation and examination room preparation, execution interventions and patient care after the procedure, expected benefits of interventions, and we mention related complications and contraindications, too. In Chapter 3.5 we write about displaying methods associated with the interventions, in particular, computed tomography and associated with the diagnostics of patophysiological indications - magnetic resonance. In the end of theoretical part we mention the role of the radiological assistant in the radiation protection od patients and workers. In the practical part we got data for quantitative research based on structured questionnaire. This way we determined the level of the patient´s education before the intervention. We showed the results including the grafic representation which are some bar charts. Then we evaluated the formulated hypotheses - Hypotheses 1, 2, 4, 5, 9 we accepted and Hypotheses 3, 5, 7, 8, we rejected. On the basis of the results of the survey, especially the results of Questions 10 and 12, we made clear and simple informative leaflet which could help with patient education, however, it is also for the general public. The informative leaflet has been distributed to the department of radiodiagnostics in The Central Military Hospital - Military University Hospital in Prague and Na Homolce Hospital and the goal of the leaflet is to improve the level of education and informations, patient cooperation during the interventions and reduced fear before the intervention.
