Vliv konceptu Bazální posturální programy na dechové funkce u dětí s dětskou mozkovou obrnou (DMO)

Abstract

Tato bakalářská práce zkoumá vliv konceptu Bazální posturální programy na dechové funkce dětí s dětskou mozkovou obrnou. Efekt tohoto terapeutického přístupu je posuzován pomocí porovnání s efektem metody míčkové facilitace Zdeny Jebavé. V kapitole Přehled současného stavu je popsána problematika onemocnění DMO spolu s onemocněními přidruženými. Jsou zde rozebrána specifika rehabilitační péče o takto nemocné pacienty. V další části této kapitoly je popsána anatomie respiračního systému, fyziologie dýchání a možnosti jeho ovlivnění pomocí respirační fyzioterapie. V metodické části je podrobně charakterizován zkoumaný soubor probandů. Dále v této části jsou uvedeny vybrané vyšetřovací a terapeutické metody využívané v kapitole nazvané Speciální část. Ve speciální části uvádíme vstupní spirometrické vyšetření obou skupin pacientů. Výstupní vyšetření každého probanda, spolu s porovnáním průměrných výsledků obou skupin, je uvedeno v kapitole Výsledky. V diskuzi uvádíme komentář k našim výsledkům a provádíme zhodnocení efektu obou metod. Výsledky ukazují, že koncept BPP může být efektivní pro zlepšení FVC a VC dětí s DMO. Dále zde uvádíme výsledky zahraničních studií zkoumajících efekt principu, na kterém je založen námi zkoumaný koncept. V závěru je zhodnocen průběh terapií a efekt námi zkoumané metody.

This bachelor thesis examines the impact of the method of Basal postural programmes on ventilation of children with cerebral palsy. The impact is evaluated based on a comparison with an effect of ball facilitation according to Zdena Jebavá. In the chapter Present state, the problematics of CP and associated diseases, are described. The specifics of a rehabilitation of children with cerebral palsy are analyzed in this part. The next part of this chapter describes the anatomy of respiratory system, respiratory physiology, and the therapeutical options respiratory physiotherapy. The methodical part describes thoroughly examined group of patients. This section also describes therapeutical and examination methods used in the chapter called Special part. In the special part, the initial spirometry results of both examined groups are presented. The final spirometry of every patient, followed by a comparison of average measurements within both groups, is presented in the chapter Results. The Discussion includes an interpretation of our results and an evaluation of the effect of both therapeutical concepts. Our results seem to indicate that the concept BPP can increase FVC and VC of children with CP. Also, the results of foreign studies examining the effect of the same principle, as Basal postural programmes are based on, are mentioned here. The chapter called Conclusion evaluates the period of data capture, therapeutic work, and the effect of our experimental method.
