Možnosti fyzioterapeutické intervence u dvou pacientů s Huntingtonovou nemocí, jako součást komplexní léčby

Abstract

Tématem této bakalářské práce je onemocnění zvané Huntingtonova choroba, ke kterému dochází důsledkem přenesení dědičného genu Huntingtin. V kapitole Přehled současného stavu je popsána anatomie jednotlivých částí mozku, historie a formy onemocnění, epidemiologie, diagnostika, klinický obraz pacienta, léčba nemoci a práce organizace Společnost pro pomoc při Huntingtonově chorobě. V části Metodika je nastíněn metodický postup, který byl zvolen pro testované pacienty. Dále vyšetřovací metody, do nichž byla zahrnuta například anamnéza, vyšetření stoje, chůze, neurologické vyšetření. Na základě vyšetření byly zvoleny terapeutické postupy, mezi které se řadí respirační fyzioterapie, senzomotorická stimulace, Frenkelovo cvičení, míčková facilitace dle Zdeny Jebavé a mobilizace. Nejdůležitější složkou Speciální částí je kazuistika, v níž je obsažen krátkodobý a dlouhodobý rehabilitační plán, který je zhotovený z výsledků kineziologického rozboru. Na tyto plány přímo navazují jednotlivé terapeutické jednotky. Závěrečnou částí práce jsou výsledky, v nichž je možné nalézt celkový výstupní kineziologický rozbor, který ve srovnání se vstupním kineziologickým rozborem hodnotí efekt terapie. Výsledky potvrzují přínos fyzioterapie na průběh léčby. V kapitole Diskuze je rozebrána problematika onemocnění a výsledky práce jsou porovnány s výsledky českých i zahraničních autorů zabývajících se HCH. Závěr hodnotí splnění cílů a přínos terapie.

The topic of this bachelor's thesis is a disease so called Huntington's disease, which occurs as a result of the transfer of the hereditary Huntingtin gene. The chapter called Overview of the current situation describes the anatomy of individual parts of the brain, the history and forms of the disease, epidemiology, diagnosis, clinical feature of the patient, treatment of the disease, moreover the chapter deals with the organization called Huntington's Disease Society. The methodology part focus on the methodological procedure that was chosen for the tested patients. Furthermore, there are examination methods, which included anamnesis, examination of standing, walking, neurological. Based on the examination were chosen therapeutic procedures, which include respiratory physiotherapy, sensorimotor stimulation, Frenkel's exercise, ball facilitation according to Zdena Jebava and mobilization. The most important theme of the special part of this thesis is the case study, which contains a short-term and long-term rehabilitation plan. It is made of the results from the kinesiological analysis. These plans are directly followed by individual therapeutic exercises. The final part of the thesis are the results in which it is possible to find the overall output kinesiological analysis, which in comparison with initial kinesiological analysis evaluates the effect of therapy. To sum up, the results confirm the contribution of physiotherapy to the treatment of Huntington's disease. The chapter Discussion discusses the issue of the disease and the results of the work are compared with the results of Czech and foreign authors dealing with HCH. The conclusion evaluates the fulfillment of goals and the benefit of therapy.
