Úrazy elektrickým proudem v přednemocniční neodkladné péči

Abstract

V mé bakalářské práci se věnuji úrazům, které byly způsobeny elektrickým proudem. V přednemocniční neodkladné péči si myslím, že tyto úrazy nejsou tak časté a zdravotničtí záchranáři s nimi nemají tak velké zkušenosti. To byl jeden z hlavních důvodů, proč jsem si toto téma zvolila ke své práci. V teoretické části se věnuji nejdříve elektrickému proudu a jeho dělení. Dále pak popisuji účinek elektrického proudu na lidský organismus a faktory určující závažnost poranění. Píšu také o lokálním působení při zásahu elektrickým proudem a možných komplikacích, které mohou nastat. V další kapitole charakterizuji laickou první pomoc, profesionální první pomoc a kardiopulmonální resuscitaci. V druhé části se zaměřuji na anatomii a fyziologii kůže. Poté shrnuji poznatky o popáleninách, jejich dělení, klasifikaci, rozsah, lokalizaci a možné komplikace popálenin. Uvádím také léčbu popálenin v přednemocniční péči zdravotnickou záchrannou službou, transport pacienta do zdravotnického zařízení a následnou péči v nemocničním zařízení. Praktická část se zabývá analýzou dokumentace zdravotnické záchranné služby při výjezdech na úrazy elektrickým proudem. Nejprve analyzuji data, která mi byla poskytnuta zdravotnickou záchrannou službou, jako jsou věk a pohlaví pacienta, dále jaký stupeň naléhavosti byl u pacienta vyhodnocen a kam byl pacient směřován. Poté posuzuji, jaké intervence byly u pacientů vykonány a jak velké či další poranění měli. Cílem bakalářské práce je zjistit stav výjezdů ZZS ÚK indikovaných k úrazům elektrického proudu a provést analýzu dat dle vytvořeného protokolu.

My Bachelors Thesis focuses on injuries caused by electric currents. I think that in pre-hospital urgent care these types of injuries are not very common and therefore paramedics do not have much experience with them. This was one of the main reasons I chose this topic for my Thesis. At the beginning of the theoretical part of this thesis I focus on electrical currents and its different types. Then I explain the effect electrical currents have on the human body and the factors that dictate the severity of the injury. I also discuss the local effects and plethora of complications that can occur when being shocked by an electrical current. Afterwards I characterize layman first aid, professional first aid and cardiopulmonary resuscitation. The later chapters of the theoretical part of this thesis focus on anatomy and the physiology of skin. I then summarize information about burns, the different types of burns, how they are classified and the range, localization and possible complications that may arise from burn injuries. I also state the treatment process for burn injuries in pre-hospital care from paramedics, how the patient should be transported to a medical facility and the subsequent treatment provided in medical facilities. The practical part of this Thesis examines paramedic documentation regarding their intervention during injuries caused by electrical currents. First, I analyze the data that was provided to me by the emergency medical service, such as the age and sex of the patient, the assessment of the severity of the injury and where the patient was transferred. Afterwards I evaluate what type of treatment the patient undertook from the paramedics, and the scale and type of secondary injuries. The goal of this part of this thesis is to find out the state of the executed interventions made by the ZZS ÚK regarding burn injuries and to analyze the data on them by using the protocol I created.
