Porovnání efektu terapie pomocí metody ACT s metodou analytického cvičení u pacientů po plastice LCA

Abstract

Tato bakalářská práce se zabývá problematikou plastiky předního zkříženého vazu a následné rehabilitace ve smyslu dvou odlišných terapií - akrální koativační terapie a analytické cvičení. Členění práce je na dvě základní části - teoretická a speciální část. V teoretické části bude stručně zmíněno o anatomii a biomechanice kolenního kloubu. Dále zde bude probrána problematika předního zkříženého vazu a možnosti terapie. Metodická část se bude věnovat kineziologickému rozboru a použitým terapeutickým metodám, což je v tomto případě akrální koativační terapie a analytické cvičení. Speciální část zahrnuje deset vstupních vyšetření každého pacienta. Probandi byli rozděleni do dvou skupin, kdy skupina A se věnovala akrální koativační terapii a skupina B analytickému cvičení. Kapitolu výsledků tvoří tabulky a grafy se vstupními a výstupními hodnotami goniometrie a svalové síly. Dále je zde uvedeno bodové hodnocení probandů Lysholm skóre. Z výsledků vyplynulo, že obě terapie, kterým se probandi věnovali, mají své klady i zápory, tudíž nelze přesně říct, která z terapií je efektivnější. U skupiny A cvičící ACT lépe vyšly výsledky goniometrie a svalové síly extensorů kolenního kloubu. U skupiny B naopak vyšly lépe výsledky svalové síly flexorů kolenního kloubu a bodové hodnocení Lysholm skóre. V kapitole diskuze jsou podrobněji rozebrány výsledky a následně porovnány s jinými studiemi. Je zde také uvedeno hodnocení efektu obou terapií. V závěru je posouzeno splnění cíle práce.

This Bachelor´s thesis deals with the issue of anterior cruciate ligament plastic surgery and subsequent rehabilitation in the sense of two different therapies - acral coactivation therapy and analytical exercises. The division of the work is into two basic parts - theoretical and special part. The theoretical part will briefly mention the anatomy and biomechanics of the knee joint. Furthermore, the issue of the anterior cruciate ligament and the possibilities of therapy will be discussed. The methodical part will be devoted to musculoskeletal assessment and used therapeutic methods, which in this case are acral coactivation therapy and analytical exercises. The special part includes 10 entrance examinations of each patient. Probands were divided into two groups, with group A practiced in acral coactivation therapy and group B practiced in analytical exercises. The chapter of results consists of tables and graphs with input and output values of joint range of motion and muscle strength. Furthermore, there is a point evaluation of probands Lysholm score. The results showed that both therapies that the probands practiced have their pros and cons, so it is not possible to say exactly which of the therapies is more effective. In group A practicing ACT, the results of goniometry and muscle strength of knee extensors performed better. In group B, on the other hand, the results of the muscle strength of the knee flexors and the Lysholm score were better. The chapter discusses the results in more detail and then compares them with other studies. There is also an evaluation of the effect of both therapies. In the end, the fulfillment of the goal of the work is assessed.
