Srovnání účinku terapie na neurofyziologickém principu a mechanické terapie na bolest dolní části zad

Abstract

Tato bakalářská se zabývá zhodnocením efektivity dvou typů terapií u pacientů s bolestmi dolní části zad v běžné klinické praxi na základě námi stanovených parametrů. Byla aplikována Dynamická Neuromuskulární Stabilizace a Mechanická Diagnostika a Terapie dle McKenzie. Pro hodnocení účinku aplikovaných terapií jsme si stanovili symptomatické, mechanické a funkční parametry. V teoretické části této práce jsou shrnuté teoretické poznatky zaměřené na bolesti dolní části zad a vymezení pojmů týkající se této problematiky. Dále byly popsány standardní fyzioterapeutické postupy, které byly při rehabilitaci aplikovány. Dále jsou popsány obě srovnávané metody. Jsou popsané jejich obecné principy a terapeutické postupy. V praktické části jsou uvedeny průběhy terapií, včetně kineziologických rozborů. U vstupního kineziologického rozboru uvedena pouze data, která se odchylují od fyziologické normy a které jsou pro bolest dolní části zad relevantní. U prvních pěti probandů byl aplikována terapie Dynamická Neuromuskulární Stabilizace. U zbylých pěti probandů byla prováděna terapie dle Mechanické Diagnostiky a Terapie dle McKenzie. V kapitole Výsledky jsou zpracované parametry terapií, které jsme stanovili a naměřili před začátkem každé terapie. Data všech probandů jsou zpracována do grafů s barevným odlišením pro jednotlivé metody. V kapitole Diskuze jsem vyhodnotila získaná data a zhodnotila efektivitu obou terapií na námi vybraném vzorku probandů. Použila jsem studie, které se dané problematiky týkají. Na základě zjištěných skutečností jsou v této části formulovány závěry a vlastní návrhy na řešení daného problému.

This bachelor thesis deals with the evaluation of effectiveness of two types of therapies in patients with low back pain in common clinical practice based on parameters which we set. We applied Dynamic Neuromuscular Stabilization and Mechanical Diagnosis and Therapy according to McKenzie. To evaluate the effect of applied therapies we determined symptomatic, mechanical and functional parameters. In the theoretical part of this work, there are findings focused on low back pain and definitions of terms concerning the issue. Furthermore, standard physiotherapeutic procedures applied during rehabilitation are described. Additionally, both compared methods are described together with their general principles and therapeutic procedures. In the practical part, processes of therapies are presented, including kinesiological analyses. For the initial kinesiological analysis we provide only the data which deviate from the physiological norm and which are relevant to low back pain. Dynamic Neuromuscular Therapy was applied in the case of the first five probands. The other five probands underwent therapy according to McKenzie's Mechanical Diagnosis and Therapy. In the section on results, there are processed parameters of therapies which we set and measured at the beginning of each therapy. Data of all probands are recorded in graphs with colour distinction for each method. In Discussion, we evaluate gathered data and effectiveness of both therapies on our sample of probands. This is done using the studies which deal with the given issue. Based on findings, conclusions are made in this section together with suggestions for solving a specific problem.
