Kovové implantáty v MR

Abstract

Tato bakalářská práce se zabývá problematikou kovových implantátů v magnetické rezonanci, se zaměřením na informovanost pacientů se zavedeným implantabilním kardiostimulátorem nebo kardioverter-defibrilátorem a možnostech či úskalích při vyšetření na MR. Teoretická část se nejprve věnuje historii vzniku a postupnému vývoji magnetické rezonanci. V dalších kapitolách je popsán princip magnetické rezonance od vzniku signálu přes relaxační čas a gradienty magnetického pole, po zobrazovací sekvence. Dále je rozebrána bezpečnost a kompatibilita při vyšetření pacientů s kovovými implantáty i fyzikální aspekty, které mohou mít vliv na vznik nežádoucích účinků při tomto vyšetření. V samostatných kapitolách 3.3.2.1. - 3.3.2.5. pojednáváme o kovových implantátech, které jsou specifické pro konkrétní medicínské odvětví a na základě publikovaných studií hodnotíme, zda se jedná o kontraindikaci vyšetření na MR či nikoliv. V rámci praktické části jsme sbírali data na kardiologické klinice FN Královské Vinohrady. Pro sběr dat byl vytvořen strukturovaný dotazník, který se soustředil na otázky ohledně informovanosti pacientů o návštěvě magnetické rezonance s integrovaným kardiostimulátorem či implantabilním kardioverter-defibrilátorem. Data jsme vyhodnotili a výsledky pro přehlednost znázornili pomocí tabulek. Následně jsme vyhodnotili stanovené hypotézy a pro lepší přehlednost doplnili o prostorové výsečové grafy. Hypotézy č. 1, 2, 3, 5, 6, 7 jsme přijali a hypotézu č. 4 zamítli. Na základě přijetí většiny stanovených hypotéz a výsledků dotazníkového šetření, jsme vytvořili stručný a jednoduchý informační leták, shrnující kroky, které je nutné podstoupit před vyšetřením na magnetické rezonanci. Druhou částí letáku je výčet nejčastěji implantovaných zařízení, které klinika kardiologie FN Královské Vinohrady využívá a pro jejíž pacienty je tento informační leták určen. Hlavním účelem informačního letáku je edukace pacientů, zvýšení míry informovanosti a zlepšení jejich spolupráce při vyšetření na magnetické rezonanci s integrovaným kardiostimulátorem nebo implantabilním kardioverter-defibrilátorem.

This bachelor thesis deals with the issue of metal implants in MRI with a focus on patient awareness with an implanted pacemaker or implantable cardioverter-defibrillator about the possibilities and pitfalls of MRI examination. At first, the theoretical part focuses on the history, formation and development of MRI. In the following chapters, the principle of magnetic resonance is described, from signal generation through relaxation time and magnetic field gradient to imaging sequences. Also, we discuss safety and compatibility during MRI examination of patients with metal implants and the physical aspects that can lead to the occurrence of adverse side effects during this examination. In separate chapters 3.3.2.1. - 3.3.2.5 we describe metal implants specific for particular medical fields, and based on already published studies, we discuss whether it is a contraindication or not. Within the practical part, we collected data at the cardiology clinic FN Královské Vinohrady. For the data collection, we used a structured questionnaire which is focused on patient awareness about an MRI attendance with an integrated pacemaker or implantable cardioverter-defibrillator. We evaluated the data and, for clarity, presented them in tables. Then, we evaluated the hypotheses, and for clarity, we presented them in graphs. Hypothesis number 1, 2, 3, 5, 6, 7 we accepted, number 4 we did not accept. Based on the acceptance of the majority of hypotheses and the results of the questionnaire, we made a brief and simple information leaflet, which summarizes the steps that must be undergone before the MRI examination. The second part of the leaflet summarizes the most commonly used implantable devices at the cardiology clinic FN Královské Vinohrady for whose patients the leaflet is intended. The main purpose of this information leaflet is the education of patients, improvement of patient awareness and bettering their cooperation before and during the MRI examination in patients with an integrated pacemaker or implantable cardioverter-defibrillator.
