Telerehabilitace po totální endoprotéze kolenního kloubu

Abstract

Tato bakalářská práce je zaměřena na problematiku telerehabilitace pacientů s totální endoprotézou kolenního kloubu a klade si za cíl porovnat její účinnost ve srovnání s běžnou ambulantní péčí v nemocničních zařízeních. V bakalářské práci jsou porovnávány dvě skupiny pacientů, každá je zastoupena 5 pacienty. Jedna skupina podstoupila rehabilitační péči konzervativním ambulantním způsobem a druhá je skupinou telerehabilitační, jež absolvovala rehabilitaci pomocí videohovorů a instruktážních videí. Instruktážní video bylo pacientům telerehabilitační skupiny předáno při vstupním vyšetření a následná terapie probíhala on-line prostřednictvím platformy Google Meet a aplikace WhatsApp. Terapie probíhala 4 týdny a následně byla ukončena výstupními vyšetřeními. Hlavním cílem práce je zjistit, do jaké míry a s jakou efektivitou lze provádět domácí rehabilitační péči u pacientů po totální endoprotéze kolenního kloubu. V úvodu se práce zabývá anatomií, kineziologií a biomechanikou kolenního kloubu, problematikou totální endoprotézy kolene a samotným pojmem telerehabilitace. V druhé části práce jsou podrobně uvedeny metody sběru dat a užité vyšetřovací postupy. Výsledným faktem je, že telerehabilitační skupina dosahovala ve většině výstupních vyšetření vyšší míry zlepšení než skupina podstupující tradiční ambulantní formu rehabilitace. Výsledkem dotazníku, který byl pacientům následně předložen, byla jejich vysoká spokojenost s průběhem telerehabilitace. V závěru je popsán celkový efekt této formy terapie včetně shrnutí jejích kladů a záporů.

This bachelor thesis is focused on the issue of telerehabilitation of patients with a total knee arthroplasty and its objective is to compare the effectiveness of telerehabilitation with standard outpatient care in medical facilities. The bachelor thesis compares two groups of patients, each of which contains 5 patients. The first group underwent rehabilitation care in a conservative outpatient manner. The second one is a telerehabilitation group which participated in rehabilitation through video calls and instructional videos. These instructional videos were handed over to the patients of the telerehabilitation group during the initial examination, and subsequently the therapy took place online via the Google Meet platform and the WhatsApp application. The therapy lasted for 4 weeks and it was terminated by final examinations. The main purpose of this thesis is to clarify to what extent and with what effectiveness can be provided home rehabilitation care for patients after total knee arthroplasty. The beginning of the thesis deals with the anatomy, kinesiology and biomechanics of the knee joint, the issue of total knee arthroplasty and also with the concept of the term telerehabilitation. The next part of the thesis describes in detail data collection techniques and examination methods which were used. The resulting fact is that the telerehabilitation group achieved higher improvement in most of the final examinations than the group undergoing traditional outpatient rehabilitation. The result of a questionnaire, which was subsequently given to the patients, was a high satisfaction with the whole course of telerehabilitation. The thesis is concluded by describing the overall effect of this form of treatment and summarising its advantages as well as its disadvantages.
