Anesteziologické informační systémy

Abstract

Anesteziologické informační systémy (AIMS) jsou kombinací softwarových a hardwarových prostředků, které představují sofistikované řešení v oblasti elektronické dokumentace a automatického záznamu průběhu anestezie. Podstatou AIMS je automatické vytvoření elektronických zdravotních záznamů (EMR), ale mohou představovat i komplexní nástroj pro řízení průběhu anestezie a podporu kvality zdravotní péče. Cílem diplomové práce je identifikovat současně používané AIMS v EU a v ČR a následně tyto systémy popsat a porovnat prostřednictvím komparativní a nákladové analýzy. Současně používané AIMS jsme identifikovali za využití observačního výzkumného šetření realizovaného prostřednictvím e-mailové a telefonické komunikace. Identifikované AIMS v ČR jsme deskriptivně popsali a následně porovnali za využití SWOT analýzy a Analytického hierarchického procesu (AHP). Výsledek AHP byl poté aplikován jako ,,efekt" do analýzy nákladové efektivity (CEA). V Evropě jsme identifikovali 13 typů používaných AIMS celkem z 13 různých států. Mezi nejčastěji identifikované AIMS patří Metavision, Centricity a ICCA. V ČR jsme identifikovali 4 nemocnice vybavené AIMS. Tyto systémy jsme deskriptivně popsali a porovnali s následujícími výsledky: Metavision - FNKV (celková bilance SWOT: 0,37 / hodnocení alternativy dle AHP: 0,49), FONS Enterprise - CKTCH Brno (0,15 / 0,23), Centricity- IKEM (-0,15 / 0,17) a Centricity - FN Plzeň (-0,47 / 0,11). Z hlediska nákladové efektivity se jako optimální systém jeví Metavision Anesthesia (FNKV) a nejméně efektivní systém Centricity (IKEM). Identifikovali jsme současně používané typy AIMS v EU i v ČR, nicméně vzhledem k nízkému počtu získaných dat z EU, nelze stanovit statisticky relevantní poměr uživatelů AIMS mezi EU a ČR. Z výsledků komparativních analýz bylo stanoveno preferenční uspořádání AIMS a závěrem bylo pro jednotlivé uživatele stanoveno doporučení z hlediska dalšího rozvoje AIMS.

Anesthesia information management system (AIMS) is a combination of software and hardware tools that represent a sophisticated solution in the field of electronic medical documentation and automatic record keepers of anesthesia conducting. The main purpose of AIMS is the automatic creation of electronic medical records (EMR), but they can also be a comprehensive tool for managing the course of anesthesia, promoting the quality of health care, and anesthesia risks reduction. The aim of this diploma thesis is to identify the currently used AIMSs in the EU and the Czech Republic and then make a description and comparison of these systems through comparative and cost analysis. We identified the currently used AIMSs using an observational research survey conducted through e-mail and telephone communication. AIMSs identified in the Czech Republic have been described and compared using SWOT analysis and Analytic Hierarchy Process (AHP) as multiple-criteria decision analysis. The AHP result was then applied as an "effect" to the cost-effectiveness analysis (CEA). A total of 13 different types of AIMSs from 13 different countries were identified in Europe. The most commonly identified AIMSs include Metavision Anesthesia, Centricity High Acuity Anesthesia, and ICCA. There were 4 identified AIMSs in the Czech Republic which we described and compared with the following results: Metavision - FNKV (total SWOT balance: 0.37 / evaluation of the alternative according to AHP: 0.49), FONS Enterprise - CKTCH Brno (0.15 / 0.23), Centricity- IKEM (-0.15 / 0.17) and Centricity - FN Plzeň (-0.47 / 0.11). In terms of cost-effectiveness, the optimal system appears to be Metavision Anesthesia (FNKV) and the least efficient Centricity system (IKEM). We identified the currently used types of AIMS in the EU and the Czech Republic. However, due to the low number of data obtained from the EU, it is not possible to determine a statistically relevant ratio of AIMS users between the EU and the Czech Republic. From the results of comparative analysis, the preferential arrangement of AIMS was determined and recommendations for individual users in terms of further development of AIMS were determined.
