Bezpečnostní informace o intervencích v procesu hodnocení zdravotnické technologie

Abstract

Léčba časného karcinomu prsu obvykle začíná operačním zákrokem, na nějž navazuje pooperační externí radioterapie (EBRT) v délce několika týdnů. Alternativou tohoto postupu je ozáření oblasti nádoru během operace (IORT) bez nutnosti dalšího ozařování. Cílem diplomové práce je vytvoření analýzy nákladové efektivity IORT pomocí přístroje Intrabeam a EBRT. Dále porovnání výsledků této analýzy s druhým modelem, do něhož navíc vstupují náklady na léčbu komplikací a zjištění, zda léčba komplikací ovlivňuje nákladovou efektivitu terapie. Analýza nákladové efektivity je provedena na základě Markovova modelu. Výsledné náklady na léčbu pomocí IORT a EBRT byly 62 412 Kč vs. 52 726 Kč a přínosy 8,883 vs. 8,925 QALY. Se započtením komplikací byly náklady 62 629 Kč vs. 52 939 Kč se stejnými přínosy. Z výsledků vyplynulo, že nákladově efektivnější metodou je EBRT, a to i v porovnání se zvýšenými náklady na léčbu komplikací.

The treatment of early stage breast cancer usually begins with a surgical procedure followed by postoperative external beam radiotherapy (EBRT) for several weeks. An alternative to the procedure described previously is irradiation of the tumor area during the surgery (IORT) without the need for further irradiation. The aim of the thesis is to create an analysis of the cost-effectiveness of IORT using the Intrabeam and EBRT devices. Furthermore, comparing the results of the analysis with the second model, in which the cost of treating complications is included so as the determining whether the cost-effectiveness of the therapy is affected by the treatment of complications, is being dealt with in the thesis. The cost-effectiveness analysis is performed on the basis of Markov's model. The resulting costs of treatment using IORT vs. EBRT were CZK 62 412 vs. CZK 52 726 and benefits of 8,883 vs. 8,925 QALY. Including complications treatment, the costs were CZK 62 629 vs. CZK 52 939 with the same benefits. The results showed that EBRT is a more cost-effective method, even compared to the increased cost of treating complications.
