Přínos laserové terapie do konceptu Mechanické diagnostiky a terapie u vertebrogenního algického syndromu

Abstract

Předložená diplomová práce se zabývá zhodnocením přínosu vysokovýkonné laserové terapie do konceptu Mechanické diagnostiky a terapie u pacientů s diagnózou vertebrogenního algického syndromu. Cílem práce je posoudit efekt u dvou různých intervenčních skupin, jedné podstupující monoterapii dle MDT a druhé podstupující kombinaci terapií vysokovýkonným laserem a MDT v celkové době sledování deseti až dvanácti týdnů. Pro vyhodnocení efektu terapií byly jako výstupy analgetického efektu sbírány hodnoty Visuální analogové škály (VAS) a pro zhodnocení efektu změny disability byly sbírány hodnoty pomocí The Oswestry Disability Index (ODI). Výsledky změn těchto hodnot byly následně statisticky ověřeny. Celkem byla retrospektivně získána data skupiny (n=80) pacientů, kteří podstoupili monoterepii (5 terapií) dle MDT. Prospektivně získaná data skupiny (n=53) pacientů podstoupila kombinaci terapií laserem (5 aplikací) a MDT (5 terapií). Výsledky diplomové práce potvrzují poukázanou efektivitu z rešerše efektivitu terapie dle MDT jako monoterapie (p-hodnota <0,05). Přínosem laserové terapie dle výsledků diplomové do konceptu MDT je možné urychlení procesu léčby až o jeden měsíc. Statisticky významné rozdíly mezi intervenčními skupinami poukázala diplomová práce ve změnách hodnot VAS mezi terapiemi, tedy v kratším sledovacím intervalu. Porovnání obou intervenčních skupin na hladině významnosti 5 % (?=0,05) v celkové době sledování 10 - 12 týdnů neprokázali statisticky významné rozdíly mezi skupinami ve sledovaných parametrech VAS (p-hodnota 0,067) a hodnotě ODI (p-hodnota 0,3). V diskuzi byly shrnuty přínosy a navržen vhodný postup kombinace terapie s vysokovýkonným laserem u pacientů s vertebrogenním algickým syndromem. Výsledky práce jsou diskutovány s autory zmíněných v rešerši.

The thesis deals with evaluation of the high-power laser therapy contribution to the Concept of Mechanical Diagnostics and Therapy (MDT) in patients with vertebrogenic algic syndrome. The aim of this thesis is to assess the effect of two different intervention groups by different approaches, while the first group underwent therapy by the MDT and the second one underwent the combination of both therapies, the MDT and high-power laser therapy. Both groups were treated for 10 to 12 weeks. To evaluate the outcome of analgetic effect, the method of the Visual Analog Scale (VAS) was chosen. To valuate disability changes, the data were collected by The Oswestry Disability Index (ODI). The results of these changes were statistically verified. In total, data from a group (n = 80) of patients who underwent only monotherapy (5 therapies) according to MDT were obtained retrospectively. The prospective data set group (n=53) underwent the combination of both therapies (a total of 5 sessions of MDT therapy and 5 of laser therapy). The results of the thesis confirm the effectiveness of the MDT method as monotherapy (p-value <0,05) which is also pointed out in the literature research. Contribution of high-power laser therapy to the concept of the MDT is perceived as a way to decrease the overall duration of the therapy about one month. The thesis showed statistically significant differences between the intervention groups in the changes of VAS values between the therapies in a shorter interval. Comparison of both intervention groups at the significance level of 5% (? = 0.05) in the total period of 10 to 12 weeks did not show statistically significant differences between the groups in the monitored parameters VAS (p-value 0.067) and ODI value (p-value 0.3). In the part of Discussion of this thesis, the benefits of high-power laser therapy are summarized and the method using the combination of the laser therapy in the patient with vertebrogenic algic syndrome is proposed. The results of the study are discussed with the clinical studies of the foreign authors mentioned in the research.
