Ekonomicko-klinické hodnocení kryoterapie u pacientů s entezopatií

Abstract

Cílem diplomové práce je provést ekonomicko-klinické hodnocení kryoterapie u pacientů s entezopatií. Diplomová práce se skládá z analýzy současného stavu, ve kterém jsou popsány aktuální trendy v léčbě entezopatií, získané z vybraných klinických studií. Následující část diplomové práce je věnována hlavním a dílčím cílům diplomové práce a metodám, které jsou aplikovány v praktické části diplomové práce. V praktické části diplomové jsou statisticky vyhodnocena klinická data, která prokazují klinický efekt léčby u pacientů s entezopatií. Následně je provedena analýza nákladové efektivity, která je ověřena jednocestnou analýzou sensitivity. Na základě analýzy nákladové efektivity z perspektivy zdravotnického zařízení můžeme považovat hodnocenou intervenci Cryogen 3 za nákladově efektivní oproti komparátoru Cry-T Elephant. Na základě výsledků nákladové efektivity z perspektivy pacienta nemůžeme u hodnocené intervence Cryogen 3 jednoznačně rozhodnout o nákladové efektivitě. Rozhodnutí je závislé na prahové hodnotě ?, která vyjadřuje hranici ochoty platit. Závěr praktické části diplomové práce byl věnován multikriteriální rozhodovací analýze technických parametrů u přístroje Cryogen 3 a Cryo-T Elephant. Na základě vypočteného relativního ukazatelé metodou TOPSIS můžeme tvrdit, že varianta Cryogen 3 je ideální variantou a varianta Cryo-T Elephant je bazální variantou.

The thesis aims to make an economically clinical evaluation of cryotherapy to patients diagnosed with enthesopathy. The diploma thesis consists to the analysis of the current state, which describes current trends in the treatment of enthesopathies obtained from selected clinical studies. The following part of the thesis is devoted to the thesis's main and fractional objectives and to the methods which are applied in the practical portion of the thesis. In the practical part of the thesis, there are statistically evaluated clinical data that prove the clinical effect of the treatment on patients with enthesopathy. Consequently, the analysis of cost-effectiveness is performed and verified by one-way sensitivity analysis. Based on the cost-effectiveness analysis from the perspective of medical devices we can consider the evaluated intervention of Cryogen 3 as cost-effective in comparison to the comparator Cry-T Elephant. Based on the results of the cost-effectiveness from the point of view of a patient, we cannot conclusively decide at the evaluated intervention Cryogen-3 about the cost-effectiveness. The decision is dependent upon the threshold value ?. This value expresses the limit of willingness to pay. The conclusion of the practical part of the thesis was devoted to the multi-criteria, decision-making analysis of technical parameters of the Cryogen 3 and Cryo-T Elephant device. Based on the calculated relative indicator, using the method TOPSIS, we can claim that Cryogen 3 is the ideal option and Cryo-T Elephant is the basal option.
