Nákladová efektivita léčby pacientů s akutním infarktem myokardu komplikovaného fibrilací komor během primární perkutánní koronární intervence

Abstract

Diplomová práce srovnává analýzu nákladové efektivity farmakologického postupu s KPR a připojení pacienta na přístroj ECMO u pacientů s akutním infarktem myokardu u nichž během perkutánní koronární intervence nastala fibrilace komor, a to v Kardiovaskulárním centru KNL. Metodou výpočtu se stala analýza nákladové efektivity s výsledným parametrem ICER. Výpočty nákladové efektivity jsou provedeny z perspektivy plátce i z perspektivy poskytovatele péče. Výsledný ICER z perspektivy plátce je roven 1 346 791 Kč/Life Saved. Výsledný ICER z perspektivy poskytovatele péče je ICER roven 1 671 662 Kč/Life Saved. Další využitou metodou je jednocestná analýza senzitivity, dále pak i pravděpodobností citlivostní analýza. Součástí analýzy je sestrojení tzv. cost-effectiveness acceptability curve (CEAC), která rozhoduje, jaké procento iterací je v určeném pásmu hodnoty hranice platit. Jako hranice je využita The Value of a Statistical Life (VSL). Nákladově efektivní se jeví hodnocená intervence ECMO.

The diploma thesis compares the cost-effectiveness analysis of the pharmacological procedure with CPR and connecting to the ECMO device patients with the acute myocardial infarction complicated by ventricular fibrillation during primary percutaneous coronary intervention at the KNL Cardiovascular Center. For the calculation method was used cost-effectiveness analysis CEA with the resulting parameter ICER. Cost-effectiveness analysis calculations are performed from the payer's view as well as from the healthcare provider's perspective. The resulting ICER from the payer's perspective equals to 1,346,791 CZK / Life Saved. The resulting ICER from the perspective of the care provider is ICER equals to 1,671,662 CZK/ Life Saved. Other methods used are the one-way sensitivity analysis, as well as the probability sensitivity analysis. Part of the analysis is the construction of the so-called cost-effectiveness acceptability curve (CEAC), which decides what percentage of iterations is in the specified range of limit to pay. The Value of a Statistical Life (VSL) is used as the limit. It appears that the Intervention in which the patients are connected to ECMO could be more cost effective.
