Radiační zátěž pracovníků během intervencí na sentinelové uzlině karcinomu prsu

Abstract

Tato bakalářská práce je zaměřena na porovnání radiační zátěže personálu při vyšetření sentinelové uzliny u karcinomu prsu. Obsahem teoretické části jsou základní informace o karcinomu prsu, jeho diagnostických a léčebných metodách. Je zde vysvětlen koncept sentinelové uzliny v závislosti na lymfatickém systému jedince. Popsány jsou i možnosti zobrazení sentinelové uzliny vitálními barvivy a radiofarmaky z hlediska práce nukleární medicíny. Závěr teoretické části práce se věnuje radiační ochraně personálu při práci s ionizujícím zářením. Praktická část se věnuje porovnání nasbíraných dat při vyšetřeních sentinelových uzlin u karcinomu prsu. Sběr dat a měření probíhala ve dvou nemocničních zařízeních s odlišným typem protokolu. Popis jednodenního a dvoudenního protokolu charakterizuje rozdílnost postupů v těchto nemocničních zařízeních. Porovnává se četnost vybraných intervencí na odděleních nukleární medicíny a množství podaných aktivit 99mTc-Senti-Scint, které pacienti při intervencích sentinelové uzliny dostali. Dalšími upřesňujícími údaji jsou věk a pohlaví pacientů. Výsledky jsou založeny na porovnání naměřené radiační zátěže lékařů nukleární medicíny a chirurgů u rozdílných protokolů. V diskusi se nachází zamyšlení nad získanými daty i srovnání dat s jinými autory. Včetně popisu možností, jak z hlediska radiační ochrany radiační zátěž personálu snížit.

This bachelor thesis is focused on comparing radiation exposure of medical staff during sentinel node examination in breast cancer. In the theoretical part, basic information about breast cancer will be discussed, including its diagnostic and treatment methods. There will also be a section explaining the concept of sentinel lymph nodes, depending on the system of the individual. It describes the possibilities of sentinel lymph node imaging with vital dyes and radiopharmaceuticals from nuclear medicine's point of view. The conclusion of the theoretical part is devoted to the description of radiation protection of personnel exposed to radiation in the course of their work. The practical part is dedicated to the comparison of collected data during sentinel lymph node examinations in breast cancer. Measurements and data collection took place in two medical facilities with different types of protocols. The differences in procedures in these hospitals are characterized by the description of one-day and two-day protocols. It contains the juxtaposed frequency of selected interventions in the departments of nuclear medicine and the amount of administered 99mTc-Senti-Scint activities that patients received during interventions. Other specifying information added are age and sex of the patients. The results are based on differences of the measured radiation exposure that nuclear medicine doctors and surgeons received for different protocols. The discussion includes reflections on the data obtained and a comparison with other authors results. This part also includes a description of how to reduce the radiation exposure of medical personnel in terms of radiation protection.
