Abstract

Zhodnocení nákladové efektivity pro screening věkem podmíněné makulární degenerace: Věkem podmíněná makulární degenerace je ve vyspělých zemích nejčastější příčinou praktické slepoty u populace starší 50 let. Toto onemocnění každoročně přináší vysoké náklady na léčbu pokročilých forem díky pomalé progresi a nezaregistrovaným subjektivním příznakům. Hlavní cílem práce této práce je provést analýzu nákladové efektivity zavedení screenigového programu populace starší 50 let pro včasný záchyt věkem podmíněné makulární degenerace (VPMD) v České republice. Dílčími cíli jsou analýza současného stavu v oblasti zatížení zrakovým postižením, diagnostiky a léčby VPMD v ČR a screeningu očních chorob v ČR a ve světě. Na základě literární rešerše dále určení klinických přínosů screeningu, kalkulování nákladů spojených se dvěma strategiemi a tyto data použít pro tvorbu modelu a následnou nákladovou analýzu. Pro splnění cílů jsou vybrány vhodné metody, které jsou aplikovány v praktické části. Porovnávána je kohorta pacientů podstupující pravidelný 5letý screening očního pozadí a jako komparátor je zvolena skupina nepodstupující screening. Je sestaven rozhodovací strom a Markovův model, vypočítána nákladová efektivita a provedena analýza citlivosti. Nákladová analýza ukazuje příznivé jak klinické, tak ekonomické přínosy screeningové strategie. Analýza citlivosti zkoumá dopady variability proměnných u strategie screeningu na výsledky CUA. Na závěr diplomové práce jsou shrnutá zjištění, výsledky analýzy i limity modelu diskutovány a jsou navrhnuta doporučení v oblasti časné diagnostiky očních onemocnění zapříčiňujících slepotu.

Evaluation of cost-effectiveness for age-related macular degeneration screening: Age-related macular degeneration is the most common cause of blindness in the population over the age of 50 in developed countries. This disease brings high costs for the treatment of advanced forms every year due to slow progression and unregistered subjective symptoms. The aim of this thesis is to analyze the cost-effectiveness of the screening program for people older than 50 years for the early detection of age-related macular degeneration (AMD) in the Czech Republic. The partial goals are the analysis of the current state in the field of visual impairment, diagnosis and treatment of AMD in the Czech Republic and screening for eye diseases in the Czech Republic and in the world. Based on the literature research, further determine the clinical benefits of screening, calculate the costs associated with the two strategies and use this data for model creation and subsequent cost analysis. To meet the goals, suitable methods are selected, which are applied in the practical part. A cohort of patients undergoing regular 5-year ocular background screening is compared and a group not undergoing screening is selected as a comparator. A decision tree and Markov model are compiled, cost-effectiveness is calculated, and sensitivity analysis is performed. The cost analysis shows the favorable both clinical and economic benefits of the screening strategy. Sensitivity analysis examines the effects of variability in screening strategy on CUA results. At the end of the diploma thesis, the findings, analysis results and limits of the model are discussed and recommendations in the field of early diagnosis of eye diseases causing blindness are proposed.
