Vyšetření Charcotovy osteoartropatie pomocí třífázové kostní scintigrafie

Abstract

Bakalářská práce se zabývá využitím třífázové scintigrafie skeletu v diagnostice a monitorování Charcotovy osteoartropatie a je rozdělena na teoretickou a praktickou část. V teoretické části je rozebrán diabetes mellitus, jeho typy a možnosti léčby. Práce se nadále věnuje jeho komplikacím ve formě syndromu diabetické nohy, konkrétně rozvoji Charcotovy osteoartropatie. Uvedena je patogeneze vzniku tohoto onemocnění, jeho klinický obraz, terapeutické a diagnostické možnosti. Dále je popsáno využití třífázové scintigrafie skeletu v diagnostice a monitorování tohoto onemocnění, včetně základního principu metody, postupů, využívaných radiofarmak a možností nálezů. V praktické části jsou prezentovány údaje o počtu a složení pacientů vyšetřovaných pro Charcotovu osteoartropatii na Klinice nukleární medicíny a endokrinologie 2. LF UK a FN Motol v letech 2015-2020. Na šesti případových studiích jsou prezentovány klinické a scintigrafické nálezy v průběhu léčby pacientů, jejich korelace a podobnost s výsledky odborných publikací.

This Bachelor thesis deals with the use of three-phase scintigraphy of a skeleton in diagnosing and monitoring Charcot's osteoarthropathy and it is divided into a theoretical and practical part. In the theoretical part diabetes mellitus, its types and possibilities of its treatment are analysed. Its complications in the form of a diabetic foot syndrome, specifically the development of Charcot's osteoarthropathy are also mentioned. As well as pathogenesis of the formation of this disease, its clinical record file, therapeutic and diagnostic ways are indicated there. Furthermore, the use of three-phase scintigraphy of a skeleton in diagnosing and monitoring this disease, including rudimentary principle of methods, procedures, used radiopharmaceuticals and possibilities of medical reports are described. In the practical part information about the number and the structure of patients examined and diagnosed as having Charcot's osteoarthropathy at the Department of Nuclear Medicine and Endocrinology, 2nd Faculty of Medicine, Charles University in Prague and Motol University Hospital in year 2015-2020 is presented. Clinical and scintigraphic reports in the process of patients' treatments, their correlation and similarities to the results published in professional journals are shown in six studies.
