Možnosti endovaskulárních intervencí na dolních končetinách

Abstract

V bakalářské práci se zabýváme terapeutickými možnostmi endovaskulárních intervencí na dolních končetinách. Práce je rozdělena na dvě části, teoretickou a praktickou. V začátku teoretické části práce je v jednotlivých kapitolách vysvětlena anatomie a patofyziologie cév dolních končetin. U každé patofyziologie je vysvětlena její etiologie. V následující kapitole jsou popsány možnosti radiodiagnostického zobrazování cév dolních končetin, včetně úlohy radiologického asistenta při diagnostické a terapeutické metodě digitální subtrakční angiografii. Další kapitolou teoretické části práce jsou pak již samotné terapeutické endovaskulární intervenční možnosti. Zde jsou podrobně vysvětleny a popsány nejčastější intervenční zákroky typických onemocnění cév dolních končetin za použití digitální subtrakční angiografie. V poslední kapitole teoretické části práce je stručně uvedena radiační ochrana při práci s ionizujícím zářením na radiodiagnostickém oddělení. V praktické části práce jsou metodou kvalitativního výzkumu zpracovány formou případových studií jednotlivé kazuistiky. Vybráno bylo deset pacientů z Ústřední vojenské nemocnice - Vojenské fakultní nemocnice Praha, u kterých jsou zpracovány detailnější kazuistiky zaměřené zejména na popis onemocnění, jeho průběh a následnou endovaskulární léčbu. Výsledkem praktické části práce je analýza, komparace a zhodnocení případových studií a průběhu samotného terapeutického výkonu.

In the bachelor's thesis we deal with the therapeutic possibilities of endovascular interventions in the lower limbs. The work is divided into two parts, theoretical and practical. At the beginning of the theoretical part of the work, the anatomy and pathophysiology of the vessels of the lower limbs are explained in individual chapters. For each pathophysiology, its etiology is explained. The following chapter describes the possibilities of radiodiagnostic imaging of the vessels of the lower limbs, including the role of a radiological assistant in the diagnostic and therapeutic method of digital subtraction angiography. Next chapter of the theoretical part of the work describes the therapeutic endovascular intervention options. Here, the most common interventions for typical lower extremity vascular diseases using digital subtraction angiography are explained and described in detail. The last chapter of the theoretical part of the thesis briefly presents radiation protection when working with ionizing radiation in the radiodiagnostic department. In the practical part of the work, individual case studies are processed by the method of qualitative research in the form of case studies. Ten patients were selected from the Central Military Hospital - the Military University Hospital in Prague, for whom more detailed case reports are prepared, focusing mainly on the description of the disease, its course and subsequent endovascular treatment. The result of the practical part of the work is the analysis, comparison and evaluation of case studies and the course of the therapeutic performance itself.
