Úloha radiologického asistenta při implantaci podkožního intravenózního portu

Abstract

V bakalářské práci se zabýváme metodou dlouhodobého zajištění žilního vstupu - implantací intravenózního portu a úlohou radiologického asistenta během tohoto výkonu. Práce je rozdělena na dvě části - teoretickou a praktickou. V úvodu teoretické části je popsána anatomie a fyziologie žilního systému horní a dolní končetiny. V další části práce uvádíme definici intravenózního port systému a popis samotného výkonu - indikace, kontraindikace, zobrazovací metody, zásady implantace, přípravu pacienta, instrumentárium, techniku implantace, komplikace, postup při extrakci a ošetřovatelské péči o intravenózní port. V následující kapitole se zabýváme úlohou radiologického asistenta před, během a po výkonu a v neposlední řadě se věnujeme radiační ochraně z pohledu personálu i pacienta. V praktické části je zpracováno metodou kvalitativního výzkumu formou případových studií deset vybraných pacientů, indikovaných k implantaci intravenózního portu. U všech pacientů byl výkon proveden v Ústřední vojenské nemocnici - Vojenské fakultní nemocnici Praha na angiografickém pracovišti radiodiagnostického oddělení. Výsledkem praktické části je detailní popis a analýza deseti vybraných případových studií, ve kterých podrobně objasňujeme jednotlivé aspekty před, v průběhu a po implantaci intravenózního portu. Následně interpretujeme jejich společné znaky, rozdílné situace vzniklé během implantace a individuální okolnosti každé kazuistiky.

In this bachelor thesis we discuss the method of long-term provision of venous access - implantation of an intravenous port and the role of a radiological assistant during this procedure. The thesis is divided into two parts - theoretical and practical. The introduction of the theoretical part describes the anatomy and physiology of the venous system of the upper and lower limbs. In the next part of the thesis we present the definition of the intravenous port system and a description of the procedure itself - indications, contraindications, imaging methods, principles of implantation, patient preparation, instruments, implantation techniques, complications, extraction procedure and nursing care of the intravenous port. In the following chapter we deal with the role of the radiological assistant before, during and after the procedure and, last but not least, we deal with radiation protection from the point of view of the staff and the patient. In the practical part, the method of qualitative research in the form of case studies is processed by ten selected patients indicated for implantation of intravenous port. All patients underwent the procedure at the Central Military Hospital - Military University Hospital in Prague at the angiographic workplace of the radiodiagnostic department. The result of the practical part is a detailed description and analysis of ten selected case studies, in which we explain in detail the various aspects before, during and after the implantation of the intravenous port. Subsequently, we interpret their common features, the different situations that arose during implantation and the individual circumstances of each case report.
