Léčba bolesti v nepřístupném terénu

Abstract

Bakalářská práce se zabývá tématem analgetik v nepřístupném terénu. Jejím cílem je vytvořit ucelený přehled možností používání analgetik v těžko přístupném terénu a seznámit odbornou i laickou veřejnost s možností využívání analgetik aplikovaných inhalační cestou Horskou službou ČR o. p. s. V teoretické části jsem vymezil kompetence záchranáře horské služby, utvořil stručný popis bolesti, uvedl možné cesty aplikace léčiv do organismu a nejpoužívanější analgetika v přednemocniční neodkladné péči a samotné léčbě akutní bolesti v neodkladné péči. Dále jsem se v práci zabýval riziky vyplývajícími z prodlení při záchranné akci v náročném terénu. Poslední kapitola je věnována inhalačnímu analgetiku - methoxyfluranu. V praktické části jsou rozebrány kazuistiky poskytnuté Horskou službou ČR o. p. s., ve kterých byl pacientům aplikován inhalační methoxyfluran z důvodu silné bolesti. Dále byla použita data z Italské studie o účinnosti methoxyfluranu. Výsledky jsou porovnány s čísly vycházejícími z kazuistik z horské služby. Zkoumaná data účinnosti methoxyfluranu podávaným Horskou službou ČR o. p. s. jsou srovnatelná s výsledky účinnosti z dat italské studie.

The bachelor's thesis deals with the topic of analgesics in inaccessible terrain. Its aim is to create a comprehensive overview of the possibilities of using analgesics in terrain which is difficult to access and to introduce to the professional and laic public with the possibility of using analgesics applied by inhalation by the Mountain service of the Czech Republic. In the theoretical part, I defined the competencies of a mountain rescuer, I created a brief description of pain, listed possible methods of application of drugs to the body, afterwards I mentioned the most commonly used analgesics in prehospital emergency care and the treatment of acute pain in emergency care. I also dealt with the risks arising from delays in rescue operations in difficult terrain. The last chapter is devoted to the inhaled analgesic - methoxyflurane. In the practical part, case reports provided by the Mountain service of the Czech Republic in which inhaled methoxyflurane was administered to patients due to severe pain are analyzed. Also data from the Italian study on the efficacy of methoxyflurane were used. The results are compared with numbers based on case reports from the mountain service. The investigated efficacy data of methoxyflurane administered by the Mountain service of the Czech Republic are comparable with the efficacy results from the data of the Italian study.
