Fyzioterapeutická intervence u extrémně nedonošeného dítěte s multifaktoriálním postižením

Abstract

V obecné části bakalářské práce je nastíněna problematika dětí s nízkou porodní hmotností, důvody předčasného porodu, komplikace u nedonošených novorozenců, jejich léčba a obecně přístup k těmto pacientům. Součástí této části je i fyziologický vývoj dítěte a popis metod z oboru fyzioterapie, které se nejčastěji u těchto pacientů uplatňují. V kapitole nazvané Metodologie práce jsou uvedeny všechny vyšetřovací a terapeutické metody, které byly ve speciální části užívány. Také je zde představeno pracoviště, na kterém terapeutické jednotky probíhaly. Speciální část se zabývá kazuistikou nedonošeného dítěte s multifaktoriálním postižením. Součástí kazuistiky je i vypracování vstupního a výstupního kineziologického rozboru, vypracování krátkodobého a dlouhodobého terapeutického plánu a na závěr i výsledky terapie. Speciální část probíhala v Dětském rehabilitačním stacionáři Zvonek v rámci Odborné praxe od 1. 4. 2019 do 9. 3. 2020. Kapitola s názvem Diskuze se zabývá úspěšností zvolených postupů. Dále je rozebrán specifický přístup k dítěti se smyslovým postižením a v neposlední řadě se část diskuze zabývá i zamyšlením nad postojem rodičů dítěte, kteří si nepřipouští míru postižení dítěte a nevěnují se terapii naplno, a jak tento přístup ovlivňuje progres pacienta.

The general part of the bachelor thesis outlines the problems of children with low birth weight, the reasons for premature birth, complications of premature new-borns, their treatment and generally how to approach these patients. This part of the thesis also includes the physiological development of the child and a description of methods from the field of physiotherapy, which are most often used with these patients. The section methodology of this thesis lists all the examination and therapeutic methods that were used in the special section. The workplace where the therapeutic units took place is also presented here. The special section undertakes the case report of a premature child with multifactorial disability. The case study includes an elaboration of input and output kinesiological analysis, elaboration of a short-term and long-term therapeutic plan and results of the therapy in conclusion. The special section took place in the Children's rehabilitation ward Zvonek as part of the training from April 1st 2019 to March 9th 2020. The chapter entitled discussion undertakes the success of selected procedures. Furthermore, a specific approach to a child with sensory disabilities is discussed and, last but not least, a part of the discussion contemplates the approach of the child's parents, who do not admit the degree of disability and do not fully engage in therapy, and how this approach affects the patient's progress.
