Porovnání účinků manuální terapie a aktivního cvičení u pacientů s ankylozující spondylitidou

Abstract

Bakalářská práce se zabývá účinky manuální terapie a aktivního cvičení u pacientů s ankylozující spondylitidou. Jsou zde popsány obecné údaje o spondylartritidách a podrobné o ankylozující spondylitidě, o jejím průběhu, patogenezi, klinickém obrazu, diagnostice, a především o současných možnostech její léčby. Je zde také popsána anatomie osového skeletu a biomechanika jednotlivých úseků páteře, jelikož páteř bývá nemocí zasažena nejvíce. Cílem práce je porovnat účinky manuální terapie a aktivního cvičení u menší skupiny probandů, a také dohledat pomocí podrobnějších studií nejnovější poznatky o možnostech ovlivnění průběhu nemoci. K dosažení cílů byla nejprve použita funkční vyšetření, která obsahovala základní vyšetření dynamiky páteře a hrudníku, a následně vyšetření podle BASMI. K vyšetření aktivity onemocnění byl využit BASDAI dotazník. Poté byly aplikovány různé prvky z manuálních technik a cvičebních metod. Speciální část obsahuje popis vstupních vyšetření u deseti probandů s ankylozující spondylitidou rozdělených do dvou skupin podle použitých terapií, které trvaly více jak tři měsíce. Následně byly provedeny výstupní vyšetření, které byly ve výsledcích porovnány se vstupními. Výsledky jsou uvedeny v přehledných grafech zobrazující porovnání účinků manuální a cvičební terapie. V diskusi jsou popsány výsledky již provedených zahraničních studií, které jsou následně porovnávány s výsledky této bakalářské práce. Zároveň jsou zde nastíněny jiné možnosti ovlivnění průběhu nemoci s nejnovějšími poznatky.

The work is focused on effects of manual therapy and active exercising on patients with ankylosing sponditilis. There are general sponditilis data described and detailed ankylosing sponditilis, its progress, pathogenesis, clinical count, diagnosis and above all, current possibilities of the treatmen, too. There are also anatomy of axial skeleton and biomechanics of individual section of the spine described because the spine is affected by the illness most. The target of the work is to compare the effects of manual therapy and active exercising in a smaller group of probands and to find the newest knowledge about possibilities of influencing of course of the disease. To reach the target the functional examinations were used. They contained basic examination of spine and chest dynamics, then the examination according to BASMI. To examine of the disease activity BASDAI questionnaire was used. Then various elements of the manual techniques and exercising methods were applied. The special part is containing description of the initial examination, of ten probands with ankylosing sponditilis who were divided into two groups according to the used therapy.It took more than three months. The output exmainations were made. Their results were compared with those of initial ones. The results are performed in clear charts showing comparison of the effect of manual and exercising therapy. In discussion results of the foreign executed studies are described and are compered with the results of this work.Together with it, the newest possibilities of influence of the course of the disease are outlined.
