Abstract

Vliv celkového astigmatismu rohovky na výpočet torické nitrooční čočky je často diskutovaným tématem pro všechny odborníky, kterých se dotýká problematika operace katarakty. V teoretické části bakalářské práce byla objasněna problematika rohovkového astigmatismu v návaznosti na anatomické struktury předního segmentu oka a jeho patologie, které na něj mají vliv. Byly popsány diagnostické metody keratometrie, topografie rohovky a biometrie oka. Tedy parametrů oka využívaných pro výpočet optické mohutnosti nitrooční čočky. Dále byla prezentována problematika aktuálních trendů kalkulace torických intraokulárních čoček. Předmětem praktické části je studie, která má za úkol porovnat dvě různé metody kalkulace torické intraokulární čočky. Barrett Toric kalkulátor pomocí svého teoretického modelu oka parametry zadního zakřivení rohovky aproximuje a vektorová (Holladay) kalkulace je do výpočtu přímo zahrnuje pomocí totální keratometrie. Porovnání bylo provedeno pomocí srovnání obou kalkulátorů s výsledky rekalkulace Barrett RX vycházející z reálné pooperační diagnostiky sledovaného souboru pacientů. Stanovená hypotéza, že kalkulace TIOL pomocí nového vzorce Barrett Toric je přesnější než kalkulace Holladay, byla potvrzena.

The influence of total corneal astigmatism on the calculation of the toric intraocular lens is a frequently discussed topic for all experts with interest in the cataract surgery.The theoretical part of the bachelor's thesis clarified the issue of corneal astigmatism in relation to the anatomical structures of the anterior segment of the eye and its pathology, which affect it. Diagnostic methods of keratometry, corneal topography and eye biometrics have been described. Thus, the parameters of the eye used to calculate the optical power of an intraocular lens. Furthermore, the issue of current trends in the calculation of toric intraocular lenses was presented. The subject of the practical part is a study, which aims to compare two different methods of toric intraocular lens calculation. Using its theoretical model of the eye, the Barrett Toric calculator approximates the parameters of the posterior curvature of the cornea and Holladay's calculation directly includes them in the calculation. The comparison was performed by comparing both calculators with the results of the Barrett RX recalculation, based on the real postoperative diagnosis of the monitored group of patients. The established hypothesis is numerically substantiated and the results were summarized in the discussion.
