ÚVOD: Transplantace ledvin je život zachraňující léčebnou metodou nejen u dospělých, ale i u dětských pacientů v terminálním stadiu selhání ledvin. Transplantaci ve většině případů předchází zahájení chronické dialyzační léčby. Ekonomická náročnost dialýzy v porovnání s transplantací ledvin nebyla dosud u dětských pacientů dostatečně zhodnocena. CÍL: Hlavním cílem práce bylo potvrdit či vyvrátit hypotézu, že náklady na chronickou dialyzační léčbu dětí jsou vyšší v porovnání s obdobím po transplantaci ledvin tak, jak bylo již opakovaně prokázáno u dospělých pacientů. METODY: V letech 2014-2018 byla ve Fakultní nemocnici v Motole provedena transplantace ledviny u 52 dětských pacientů. Z tohoto souboru byla hodnocena skupina 33 dětských pacientů, kteří podstoupili transplantaci ledvin v uvedených letech a splnili kritérium možnosti hodnocení nákladů 6 měsíců před transplantací a 6 měsíců po transplantaci. 1 pacient byl vyřazen pro extrémně vysoké náklady na léčbu eculizumabem v předtransplantačním období. 27 pacientů z této skupiny podstoupilo dialyzační léčbu před transplantací, 6 pacientů podstoupilo preemptivní transplantaci. Pro vyčíslení nákladů na léčbu byla použita bodová vyhláška 357/2017 Sb., aby náklady nebyly zkreslovány inflací. Pro statistické vyhodnocení a tvorbu grafů byl použit statistický balík R. VÝSLEDKY: Bylo prokázáno, že po transplantaci dochází v celém souboru ke statisticky významnému snížení nákladů. Před transplantací dosahovala průměrná cena léčby na jednoho pacienta 434 780 Kč, zatímco půl roku po transplantaci náklady klesají na částku 237 908 Kč. Průměrné náklady na straně ZP na transplantaci ledviny jsou 379 357 Kč. Podařilo se prokázat, že v celém hodnoceném souboru dětských pacientů dochází po transplantaci ledviny k výraznému snížení nákladů na zdravotní péči na straně zdravotní pojišťovny. Při bližším zkoumání bylo zjištěno, že tento trend je zásadně ovlivněn skupinou dialyzovaných pacientů, u kterých k tomuto snížení dochází výrazně. Naopak, u pacientů, kteří byli transplantováni preemptivně, tedy bez předchozí dialyzační terapie, analyzovaná data o změně výše nákladů nesvědčí a bylo prokázáno zvýšení nákladů prvním půl roce po transplantaci, což lze vysvětlit zvýšením nákladů bezprostředně po transplantaci oproti nízkým nákladům u nedialyzovaných pacientů před transplantací. Zároveň bylo zjištěno, že náklady na dialyzované pacienty před transplantací ledviny jsou signifikantně vyšší než na pacienty transplantované preemptivně, a to jak před transplantací, tak po transplantaci. ZÁVĚR: Výsledky potvrzují hlavní hypotézu práce, že transplantace ledvin u dětských pacientů je finančně méně nákladná než léčba dialýzou. Vedle medicínských důvodů tento fakt významně podporuje časnou indikaci transplantace ledvin u dětí v terminální fázi selhání ledvin. INTRODUCTION: Kidney transplantation is a medical procedure used not only for adults, but also for pediatric patients with end-stage renal failure. In most cases, a chronic dialysis treatment precedes the renal transplant itself. The costs of dialysis versus costs of renal transplantation for pediatric patients have not been sufficiently compared in research yet. OBJECTIVE: The main goal of this thesis was to prove or disprove a hypothesis which states that the costs of chronic dialysis treatment are higher than the costs of renal transplantation for pediatric patients. The same hypothesis has been repeatedly proved to be applicable for adults. METHODS: The research sample consists of a total of 52 pediatric patients who underwent renal transplantation during the period 2014-2018 in Motol University Hospital. 33 of these pediatric patients underwent a transplantation in years 2014-2018 and met the criteria for evaluating the costs 6 months before the transplant and 6 months after. 1 patient was excluded from the sample due to extra high costs of treatment prescribing eculizumab during the period prior to the transplantation. 27 patients underwent a dialysis treatment before transplantation and 6 patients underwent preemptive transplant directly. In order to assess the costs of medical treatment precisely - without distortion due to inflation - the regulation 357/2017 Sb. has been applied. The R package is used for statistical analysis and diagram creation. RESULTS: It was proved that after the transplantation, there is a statistically significant decrease in expenditure for the whole sample. Before the transplantation, the average cost of treatment for one patient was 434 780 CZK, whereas a half year after the transplantation, the cost was reduced to 237 908 CZK. The average cost of treatment for health insurance to cover the kidney transplantation is 379 357 CZK. We managed to prove that in the whole sample of pediatric patients there is a significant reduction in treatment costs for the health insurance side after the transplantation. During a closer examination, it was discovered that this trend is distinctly influenced by the group of dialysis patients, where the cost reduction is most prominent. Conversely with patients that have had a preemptive transplantation, without a previous dialysis therapy, the analyzed data does not show a change in treatment cost, and it was proved that there is an increase in treatment costs in the first six months after the transplantation, which can be explained by the increase in expenditure immediately after the transplantation as compared to the low expenditure from patients not on dialysis before the transplantation. It was also discovered that the treatment costs for patients with dialysis therapy before the kidney transplantation are significantly higher than the treatment costs for patients who undergo the transplantation preemptively, both before and after the transplantation. CONCLUSION: The results confirm the main hypothesis of this thesis, which states that kidney transplantation for child patients is more cost effective than treatment with dialysis. Apart from medical reasons, this fact strongly supports an early indication of kidney transplantation in child patients with end-stage kidney failure.
