Mikro-simulační modely v hodnocení zdravotnických technologií

Abstract

Předmětem diplomové práce bylo použít techniku mikro-simulačních modelů pro odhad nákladové efektivity stávajícího screeningového programu zaměřeného na kolorektální karcinom v České republice. V praktické části byl využit mikro-simulační model zvaný CMOST (Colon Modeling Open Source Tool), který byl vyvinut a implementován pro modelování přirozené historie kolorektálního karcinomu, simulaci různých screeningových intervencí a výpočtu nákladů. Získané výsledky byly poté použity do analýzy nákladové efektivity a analýzy scénářů. Při zvolené populaci 100 000 se celkové náklady komparátoru a intervence značně lišily. Celkové náklady komparátoru byly celkem 3 270 180 000 Kč s efektem ztracených let života 37 099,75, zatímco intervence vyšla na 2 573 750 000 Kč s efektem 24 055,8 let. Ze získaných výsledků je patrné, že intervence je levnější o 696 430 000 Kč a ztratíme také méně let života. Výsledek diplomové práce poukazuje na to, že screeningové nastavení je ekonomicky výhodnější a zároveň přináší lepší efekt než komparátor, tak jak tomu bylo v již původním nastavení.

The subject of the diploma thesis was to use the technique of micro-simulation models to estimate the cost-effectiveness of the current screening program focused on colorectal cancer in the Czech Republic. In the practical part, a micro-simulation model called CMOST (Colon Modeling Open Source Tool) was used, which has been developed and implemented to model the natural history of colorectal cancer, simulate various screening interventions, and calculate costs. The obtained results were then used in the cost-effectiveness analysis and the scenario analysis. With a population of 100,000, the total cost of the comparator and the intervention varied considerably. The total cost of the comparator was a total of CZK 3,270,180,000 with the effect of lost years of life 37,099.75, while the intervention cost CZK 2,573,750,000 with the effect of 24,055.8 years. The obtained results show that the intervention is cheaper by CZK 696,430,000, and we will also lose fewer years of life. The result of the diploma thesis points out that the screening setting is more economically efficient, and at the same time brings a better effect than the comparator, as was the case in the original setting.
