Programy péče pro chronicky nemocné : výzva pro český zdravotní systém?

Abstract

Cílem diplomové práce je zhodnotit přínos a podporu eventuálnímu zavádění disease management programů (DMP) v ČR. DMP cílí na zlepšení řízení léčby chronických pacientů a díky lepší koordinaci péče snížit náklady na jejich léčbu. V práci byla hodnocena situace ve Spojených státech, jako v místě vzniku, a v dalších třech evropských státech: v Německu, v Nizozemsku a ve Švýcarsku. Následně byla zhodnocena současná situace léčby chroniků v České republice a na Slovensku. Na základě těchto zjištění byl navrhnut systém na zlepšení současné nepříznivé situace chronických pacientů. Konceptuálním vzorem se stal Zdravotní plán Medipartner od společnosti KlientPRO. Zavedení Zdravotního plánu s obsaženým DMP bylo shledáno jako možné řešení lepší koordinace péče, a to i jak chronických, tak zdravých pacientů v rámci jejich preventivní péče a aktivnímu přístupu k vlastnímu zdraví.

The purpose of this thesis is to evaluate the benefit and support of eventual introduction of disease management programs (DMP) in the Czech Republic. DMP strive to improve the management of treatment for chronic patiens and to reduce the costs of their treatment by better coordination of healthcare. The thesis evaulates the situation in the USA, as the country of origin, and in three European countries: Germany, the Netherlands and Switzerland. After that, the current situation of chronic treatment in the Czech republic and Slovakia was evaluated. Based on the findings, the system for better situation of chronic patients was proposed. The health plan Medipartner from KlientPRO served as a conceptual model. Introduction of this system can solve bad situation of chronic patients and health patients as part of their preventive care and active approach to their own health.
