Hodnocení analgetického efektu vysokovýkonné laserové terapie při poruchách pohybového aparátu (observační studie)

Abstract

Diplomová práce se zabývá analgetickým účinkem vysokovýkonné laserové terapie (HPLT) a jejího optimálního využití pro efektivní poskytovaní péče v ambulantním rehabilitačním prostředí. Cílem bylo vytvoření a vyhodnocení průběhu observační studie za účelem ověření analgetického účinku HPLT u pacientů s vertebrogenním algickým syndromem a jeho vliv na celkovou délku a efekt komplexní léčebně rehabilitační péče o tyto pacienty. V teoretické části práce byl shrnut současný stav problematiky systematickou literární rešerší. Ze zahraničních studií byly identifikovány metody hodnocení bolesti, designy studií, možnosti komparátorů a výsledky pro porovnání. Observační studie měla pro hodnocení dva zdroje informací. Jedním z nich byl získaný soubor dat (n=536) vyhodnocovaný retrospektivně a druhým byl sběr v rámci prospektivní studie (n=156), ve kterém byli probandi rozděleni do tří skupin. Na základě poznatků ze statistického zpracování retrospektivních dat, byly stanoveny hypotézy, které byly ověřovány z výsledků prospektivního sběru dat. Metodou pro hodnocení pociťované bolesti byla zvolena vizuální analogová škála (VAS). Probandům v první intervenční skupině (n=49) byla aplikována premedikace 4 aplikací HPLT v intervalu do 14dní před nástupem na manuální terapii. Druhá skupina pacientů absolvovala premedikaci 4 aplikací HPLT v delším časovém intervalu než 14 dní. Kontrolní skupina (n=50) absolvovala pouze manuální terapii. Hypotézy byly testovány pomocí dvouvýběrových t-testů, na základě výsledků F-testu o shodě rozptylu. Z výsledků prospektivní studie byl prokázán analgetický efekt HPLT (p=0,00001) i jeho pozitivní vliv v rámci komplexní terapie (p=0,00002). Dále bylo statisticky ověřeno, že při častější aplikaci se dosahuje efektivnějšího tlumení bolesti (p=0,00001), než při delších intervalech mezi jednotlivými aplikacemi. V neposlední řadě byl prokázáno, že premedikace HPLT statisticky významně zkracuje celkovou délku trvání komplexní terapie (p=0,00001). Na základě poznatků z observační studie byla v diskuzi navrhnuta metodika léčebného postupu péče o pacienty s vertebrogenními obtížemi, která zvyšuje o 23 % efektivitu léčby při tišení bolesti pacientů a zkracuje dobu jejich léčby o 25 procent. Výsledky byly v souladu s klinickými studiemi zahraničních autorů.

The thesis is devoted to the evaluation of the analgesic effect of high-intensity laser therapy (HPLT) and optimization of its utilization in outpatient rehabilitation environment. The aim of this work was to prepare, carry out and evaluate the observational study focused on the verification of the analgesic effect of HPLT on patients with vertebrogenic algic syndrome and its effect on the overall length and results of comprehensive rehabilitation care of these patients. The theoretical part of the thesis summarizes the state of the definition of HPLT based on the literature review. Methods of pain assessment, study designs, compactor options, and comparison results were analyzed based on foreign studies. The study had two sources of information for evaluation. One of them was the retrospective data set (n=536) and the second was a prospective observational study (n=156), where they were divided into three groups. Hypothesis for the prospective study was established based on the findings from the statistical processing of retrospective data. As the method for evaluating the pain felting was chosen the method of the Visual Analog Scale (VAS). The first intervention group (n=49) had a premedication of 4 applications of HPLT at intervals within 14 days or less before manual therapy. The second intervention group had a premedication of 4 HPLTs with an interval of more than 14 days. The control group (n=50) was observed only by manual therapy. Hypotheses were testing using a two-sample t-test and based on results of F-test`s variance. The results of the prospective study demonstrated the significant analgesic effect of HPLT (p=0,00001) and its positive effect within the complex therapy (p=0,00002). In addition, it was statistically confirmed that more effective pain control is achieved (p = 0.00001) in a short-term than at longer intervals between applications. Finally, HPLT premedication has been shown to significantly shorten the overall duration of complex therapy (p=0,00001). Based on these results, an approach for the treatment of patients with vertebrogenic disorders was proposed, which increases treatment efficacy by up to 23% in patients pain relief and reduces treatment time by 25 percent. The results were consistent with the clinical studies of foreign authors.
