Úloha radiologického asistenta při nevaskulárních intervencích na žlučových cestách

Abstract

Tato bakalářská práce se zabývá nevaskulárními intervenčními výkony na žlučových cestách a úlohou radiologického asistenta během těchto výkonů. Studovaným tématem je průběh intervenčních výkonů a jejich terapeutický přínos pro pacienta. Práce se skládá ze dvou částí, teoretické a praktické. V úvodu teoretické části je popsána anatomie žlučových cest a základní zobrazovací metody, které se využívají při zobrazování žlučových cest. Především to jsou ultrasonografie, skiagrafie, výpočetní tomografie, skiaskopie, magnetická rezonance a další diagnostické modality. Součástí teoretické části je dále výklad nejběžnějších onemocnění biliárního systému. Na tyto pak navazuje popis nevaskulárního intervenčního zákroku - perkutánní transhepatické drenáže, příprava pacienta, indikace, kontraindikace a obecně úloha radiologického asistenta při nevaskulárních intervencích. Poslední kapitolou je radiační ochrana. V praktické části jsou metodou kvalitativního výzkumu formou případových studií (kazuistik) představeni vybraní pacienti indikovaní k perkutánní transhepatické drenáži. Všechna data, jak textová i obrazová, byla získána z nemocničního informačního systému Ústřední vojenské nemocnice - Vojenské fakultní nemocnice Praha a systému PACS tamtéž. Výsledkem praktické části je analýza, komparace a zhodnocení deseti případových studií, interpretace společných a rozdílných činitelů onemocnění a průběhu samotného intervenčního výkonu - perkutánní transhepatické drenáže.

This bachelor thesis discuss with non-vascular interventional procedures on bile ducts and the role of radiological assistant during these procedures. The studied topic is the course of intervention procedures and their therapeutic benefit for the patient. The thesis consists of two parts, theoretical and practical. The introduction of the theoretical part describes the bile duct anatomy and basic imaging methods used in bile duct imaging. Above all, ultrasonography, skiagraphy, computed tomography, fluoroscopy, magnetic resonance, and other diagnostic modalities. The theoretical part also explains the most common diseases of the biliary system. These are followed by a description of non-vascular intervention - percutaneous transhepatic drainage, patient preparation, indications, contraindications, and generally the role of radiological assistant in non-vascular interventions. The last chapter is radiation protection. In the practical part, selected patients indicated for percutaneous transhepatic drainage are presented by the method of case studies by qualitative research. All data, both textual and pictorial, were obtained from the hospital information system of Ústřední vojenská nemocnice - Vojenská fakultní nemocnice Praha and the PACS system. The result of the practical part is the analysis, comparison and evaluation of case studies, interpretation of common and different factors of the disease and the course of intervention procedure itself - percutaneous transhepatic drainage.
