Plicní ventilace v akutní péči

Abstract

Bakalářská práce se zabývá problematikou chronické obstrukční plicní nemoci a jejím zaléčením v přednemocniční neodkladné péči. V teoretické části je popsána anatomie horních a dolních dýchacího systému, fyziologie, dechové objemy a kapacity plic. Dále je popsána stabilní forma chronické obstrukční plicní nemoci, její rizikové faktory, diagnostické metody, symptomy, klinické fenotypy a klasifikace. V neposlední řadě jsou zde také rozebrány čtyři kroky terapie, farmakologické i nefarmakologické a specifická léčba o jednotlivé fenotypy. Teoretická část se mimo jiné zabývá také umělou plicní ventilací. Rozebírá její fyziologické a klinické cíle, indikaci k jejímu užití a také formy, z nichž je podrobněji popsána pouze ventilace pozitivním přetlakem. Dále pojednává o kontraindikacích, komplikacích a podmínkách ukončení umělé plicní ventilace. Praktická část je zaměřena na kazuistiky s výzvou pro dušnost, kde je sledován postup zdravotníků od příjezdu k pacientovi, až po jeho předání do zdravotnického zařízení.

The bachelor's thesis deals with the Chronic Obstructive Pulmonary Disease (COPD) and its treatment in emergency care. The theoretical part of the thesis describes the anatomy of the upper and lower respiratory tracts, the physiology, pulmonary volumes and pulmonary capacity. The thesis also covers the stable form of COPD, the risk factors, diagnostic methods, symptoms, clinical phenotypes and classification. Last but not least, the theoretical part provides an analysis of a four-step therapy, including the pharmacological and non-pharmacological therapy and specific treatment of individual phenotypes. Apart from that, the artificial pulmonary ventilation is addressed, focusing on its physiological and clinical targets, the indication for use and its forms, with a special focus on the positive pressure ventilation. The theoretical part also discusses the complications and undesirable effects of artificial pulmonary ventilation. The research part analyses case studies of the emergency medical service treating patients suffering from dyspnea, focusing on how the paramedics treated the patient from their arrival on the scene to the point of checking the patient in a health-care centre.
