Role zdravotnického záchranáře v problematice zajišťování intraoseálního vstupu

Abstract

V teoretické části bakalářské práce jsou předloženy ucelené informace o problematice intraoseálního vstupu, které pochází z analýzy současné literatury. Krátce je zde nastíněn vývoj metody, od prvních zkušeností s aplikací, až po její současnou roli v zajištění přístupu do cévního řečiště v praxi. Pro lepší uvedení do problematiky slouží přehled anatomicko-fyziologických souvislostí. Do uvedené části bakalářské práce jsou zahrnuty i indikace a kontraindikace intraoseálního vstupu a jeho nejčastější komplikace, dále v současnosti užívané pomůcky a postupy k zajištění intraoseálního vstupu a popsáno je i jeho používání v širších souvislostech a situacích. Praktická část je zaměřena především na reálné využívání intraoseálního vstupu a úskalí jeho aplikace v praxi zdravotnických záchranářů v rámci Ústeckého kraje. Zaznamenán je stav znalostí záchranářů o problematice a současně je mapována jejich osobní zkušenost s aplikací tohoto vstupu v podmínkách reálné praxe. Taktéž jsou předložena zjištění o nejčastějších důvodech k jeho zavedení, ale pozornost je věnována i výskytu komplikací a dalších aspektů, souvisejících s používáním uvedené metody. Výzkum probíhal pomocí polostrukturovaných rozhovorů se záchranáři, kteří pracují jak na výjezdových základnách zdravotnické služby v rámci přednemocniční neodkladné péče, tak i se záchranáři pracujícími na urgentních příjmech při poskytování akutní intenzivní péče. Bakalářská práce tak podává ucelený přehled o problematice reálné praxe zdravotnických záchranářů konkrétních cíleně vybraných pracovišť. Tím, že potřebná data byla získána cestou polostrukturovaného rozhovoru se záchranáři, kteří pracují na výjezdových základnách zdravotnické záchranné služby a záchranáři z urgentních příjmů v nemocnicích, je bakalářská práce obohacena o srovnání zkušenosti záchranářů v problematice intraoseálního vstupu dle rozdílu v charakteru poskytované péče. Výzkumné šetření je doplněno kazuistikami pacientů, jejichž zdravotní stav a situace vyžadovaly zajištění intraoseálního vstupu v průběhu poskytování zdravotní péče na urgentním příjmu, či v přednemocniční péči a které dokladují důvody použití intraoseálního vstupu v rámci KPR. Výsledky obou částí bakalářské práce posloužily nejen k vymezení role zdravotnického záchranáře při aplikaci intraoseálního vstupu, ale také k vypracování stručných doporučení pro praxi tak, aby byla minimalizována rizika, která sebou řešený parenetrální vstup nese.

The bachelor thesis clearly presents comprehensive information on intraosseous access in the theoretical part which comes from the analysis of contemporary literature. There is briefly mentioned a method development from its first experience with application to current role in administering access to the bloodstream in practice. For a better introduce of this topic there is an overview of the anatomical-physiological context. During that part of bachelor thesis are therefore included information about this kind of parental approaches such as indications and contraindications of intraosseous access and further currently used tools and procedures to administer intraosseous access and its use in wider aspects and situations are there also mentioned. The practical part is mainly focused on the utilization of intraosseous access in reality and issues connected with its applications in practice of rescuers of Ústí nad Labem Region. In that field, the state of knowledge of rescuers is recorded, and, at the same time, their personal experience with the application of this procedure under the conditions of real practice is monitored. Furthermore, findings of most occurred reasons are presented, although attention is also focused on complications occurrence and other aspects all connected to the usage of a given method. The research was conducted using semi-structured interviews with both rescuers working at the outbound health care outposts in pre-hospital emergency care as well as emergency rescue workers in providing intensive acute care. The bachelor thesis then gives a comprehensive overview of the problems of real practice of medical rescuers of specifically targeted workplaces. The fact that the necessary data was obtained by means of a semi-structured interview with both rescuers who work at the emergency medical service outposts and rescuers from urgent care in hospitals is thus enriched by a comparison of the experience of rescuers in the issue of intraosseous access according to the difference in the nature of the provided care. The research is complemented by case reports of patients whose medical condition required providing the intraosseous access during the administration of health care on urgent care or in pre-hospital care, which substantiates the reasons for the use of intraosseous access within CPR. The results of both parts of the thesis served to define the role of the paramedic in the application of intraosseous entry, but also to develop brief recommendations for practice to reduce the risks of using this kind of parenteral access.
