Rehabilitace po totálni endoprotéze kyčelního kloubu s pomocí telerehabilitace

Abstract

Tato bakalářská práce se zabývá srovnáváním a zhodnocováním účinků rehabilitace u dvou skupin pacientů po totální endoprotéze kyčelního kloubu. První skupinu jsou pacienti, kteří rehabilitovali ve zdravotnickém zařízení pod odborným dohledem terapeuta. Druhou skupinu tvoří pacienti, kteří rehabilitovali v domácím prostředí za pomocí instruktážního videa, a komunikace s terapeutem probíhala prostřednictvím video-komunikačních technologií. V každé skupině byli tři pacienti, kteří byli vybrány náhodně. Pacienti obou skupin absolvovali vstupní vyšetření. Skupina pacientů rehabilitujících v domácím prostředí obdržela po vstupním vyšetření edukační video. Terapie s touto skupinou byla uskutečněna prostřednictvím internetu, konkrétně přes aplikaci Skype po dobu sedům týdnu. Po ukončení terapie každý z probandů prošel výstupním vyšetřením. Cílem této bakalářské práce bylo zjistit, jestli je možné využitím telerehabilitace dosáhnout stejných, nebo porovnatelných výsledků jako při běžné rehabilitaci. První část práce pojednává o základní anatomii kyčelního kloubu, jeho kinetice a kinematice. Poté je zmíněná aloplastika kyčelního kloubu, typy endoprotéz, základní operační přístupy, indikace a kontraindikace k operaci. Dále je shrnutá rehabilitace a popsán termín telerehabilitace. V další části práce jsou uvedený vyšetřovací a terapeutické metody, a popis cviků, které jsou předmětem instruktážního videa. Vyšetřovací metody byli využitý ke shromáždění údajů o pacientech. Speciální část obsahuje základní údaje o pacientech, jejích vstupní a výstupní kineziologické rozbory. V závěrečné části jsou shrnutý výsledky práce a porovnání účinnosti běžné rehabilitace s telerehabilitací.

This bachelor thesis compares and evaluates the effects of rehabilitation in two groups of patients after total hip arthroplasty. In the first group are patients who have rehabilitated in specific healthcare facility, under the supervision of a therapist technician. The second group includes people who have been rehabilitating in home environment with online and video communication. There were three patients in each group that were randomly selected and were given an introductory examination. A group of patients rehabilitating at home received an educational video after the initial examination. The whole therapy with the second group was supervised over the internet communication specifically through skype app to provide additional guidance by their therapist technician. The therapy of both groups lasted for seven weeks. At the end, all of the attendants from each group underwent the final examinations. The aim of this paper was to observe whether it is possible to achieve same or comparable results using tele rehabilitation, like it was used in the second group, in comparison to classical rehabilitation, used in the first group. The first part deals with the basic anatomy of the hip joint, its kinetics and kinematics. After, there is written about alloplasty of the hip joint, types of endoporotheses, basic surgical approaches, indications and contraindications to a surgery. Further, there is described rehabilitation and the term tele-rehabilitation itself. In the next part of the paper are investigative and therapeutic methods, plus description of exercises that are the subject of instructional video. There is specific part containing basic data about patients, their before and after kinesiology tests. Examining methods were used to collect patients' data. The final part summarizes the results of the work and compares the effectiveness of classical rehabilitation with tele-rehabilitation.
