Účinnost metody McKenzie při léčbě bolesti bederní páteře z pohledu pacienta

Abstract

Hlavním tématem této bakalářské práce se stala prezentace speciální rehabilitační metody nazvané Mechanická diagnostika a terapie (dále jen ,,MDT"). Tato metoda představuje mezinárodně uznávanou metodu vyšetření a speciální rehabilitační léčbu bolesti páteře a periferních kloubů. Teoretická část bakalářské práce je věnována anatomii zkoumané postižené oblasti bederní páteře. Tato část se dále zaměřuje na nejčastější příčiny bolesti, jednotlivé symptomy a možnosti léčby MDT metodou. V úvodu praktické části jsou obsaženy kazuistiky vzorku čtyř vybraných pacientů včetně předepsaných vyšetřovacích protokolů a sepsaných anamnéz. Zjištění účinnosti léčby formou MDT metody je následně zdokladováno na základě rozboru výsledků dobrovolně vyplněných dotazníků ze strany pacientů, kteří se účastnili terapie v rámci individuálního rehabilitačního plánu. Porovnáním dat vstupních a výstupních vyšetření je zároveň ověřeno, že MDT metodou lze skutečně dosáhnout částečného či výrazného zlepšení obtíží pacienta a současně obnovení požadovaných funkcí bederní části páteře, aniž by bylo nutné přistoupit k aplikaci medikamentózní léčby či k chirurgickému zákroku. Ze závěru bakalářské práce vyplývá, že pro pacienta je přínosnější, dosáhne-li fyzioterapeut při jeho léčbě srovnatelného zlepšení obtíží touto neinvazivní metodou. Tento fakt je zřejmý nejen z hlediska potenciálních komplikací spojených s operačním zákrokem, ale také proto, že se do budoucna zásadně změní pacientův subjektivní náhled na příčiny bolestí a možnosti efektivní autoterapie.

The main topic of this bachelor thesis is a presentation of a special rehabilitation method called Mechanical Diagnosis and Therapy (hereinafter referred to as MDT). This method is an internationally recognized method of examination and special rehabilitation treatment of spine pain and peripheral joints. The theoretical part of the thesis is devoted to an anatomy of an affected area of a lumbar spine. This section also focuses on the most common causes of pain, individual symptoms and treatment options for MDT. The introduction of the practical part contains case reports of a sample of four selected patients including prescribed examination protocols and medical histories. The assessment of effectiveness of MDT treatment is subsequently documented on the basis of the results of voluntarily completed questionnaires from the patients who participated in the therapy within the individual rehabilitation plan. At the same time, by comparing the input and output data, it is verified that the MDT method can indeed achieve a partial or significant improvement in the patient's difficulties as well as restoring the required lumbar spine functions without a need for drug treatment or surgery. From the conclusion of the bachelor thesis results that it is more beneficial for a patient if a physiotherapist achieves a comparable improvement with this non-invasive method in his therapy. This fact is evident not only from the point of view of potential complications associated with surgery, but also because the patient's subjective view of causes of pain and the possibility of effective autotherapy will change fundamentally.
