Podpora self-managementu v rámci léčby pacientů

Abstract

Diplomová práce se zabývá problematikou podpory self-managementu (SM) v rámci léčby pacientů s diagnózou diabetes mellitus I. a II. typu. Obsahuje shrnutí dostupných poznatků ze zahraničních publikací a popisuje zkušenosti s tímto konceptem. Dále analyzuje zakotvení SM v českém i zahraničním systému zdravotní péče. S prodlužující délkou života a nárůstem výskytu chronických onemocnění se neustále zvyšují náklady spojené s léčbou pacientů. Self-management rozšiřuje dovednosti nemocného při řešení zdravotních problémů a přispívá k výraznému zlepšení kvality jeho života. Cílem diplomové práce je zjistit míru aktivace/zapojení pacientů v rámci udržení a zlepšení jejich zdravotního stavu na diagnóze diabetes mellitus, což vede k zefektivnění kvality zdravotnické péče a snížení nákladů zdravotnickým zařízením. Pro správné začlenění SM do léčby je nutný kooperativní přístup pacienta, jeho rodinných příslušníků, lékařů a dalších odborníků z oblasti zdravotnictví.

The diploma thesis deals with the issue of support of self-management (SM) in the treatment of patients diagnosed with diabetes mellitus I and II. type. It contains a summary of the available knowledge from foreign publications and describes the experience with this concept, it also analyzes SM in the Czech and foreign health care systems. With increasing life expectancy and increasing incidence of chronic illness, the cost of treating patients is steadily increasing. self-management extends the patient's skills in solving health problems and contributes to a significant improvement in the quality of life. The aim of the diploma thesis is to find out the degree of pacient activation in maintaining and improving their health status on the diagnosis of diabetes mellitus, which leads to the improvement of the quality of health care and the reduction of costs for healthcare facilities. In order to properly incorporate SM into treatment, a cooperative approach is needed for the patient, his / her family members, physicians and other healthcare professionals.
