Abstract

Nákladová efektivita včasné rehabilitace pacientů po cévní mozkové příhodě Práce zpracovává problematiku včasné rehabilitace po cévní mozkové příhodě (CMP). Cílem práce je na základě dat získaných z proběhlé klinické studie analyzovat klinické efekty terapie, náklady na hospitalizaci a porovnat nákladovou efektivitu pro skupiny pacientů dle tíže postižení. Výsledky práce prokázaly vazbu skóre z testu Barthelové a Testu funkční soběstačnosti na kategorii pacienta v ústavní péči dle Seznamu zdravotních výkonů. Průměrné celkové náklady na hospitalizaci na včasném rehabilitačním lůžku byly z perspektivy poskytovatele vyčísleny na 114 489 Kč, průměrné náklady přepočítané na den pak činí 5 104 Kč. Analýza nákladové efektivity vychází z kategorií dle Seznamu zdravotních výkonů a jako nejefektivnější stanovuje skupinu částečně soběstačných pacientů. Vazba mezi funkčním deficitem pacienta a výslednými náklady na hospitalizaci byla prokázána a součástí práce je na základě tohoto vztahu také doporučení pro systém úhrad DRG v závislosti na kategorii pacienta v ústavní péči.

Cost effectiveness of early rehabilitation of patients after stroke The thesis deals with the problems of early rehabilitation after stroke. The thesis, based on the facts obtained from a recent clinical study, aims at analysing the clinical effects of the therapy, the cost of hospitalization, and comparing cost effectiveness for different groups of patients according to the seriousness of their disability. The results of the study proved the connection of the Barthel Index score and the Functional Independence Measure with the category of patients in institutional care according to the List of medical procedures. The average cost of hospitalization at early rehabilitation ward from the provider's point of view was assessed at 114 489 CZK, the average cost per day at 5 104 CZK. The cost effectiveness analysis is based on the categories according to the List of medical procedures, and determines the group of partially self-sufficient patients as the most effective. The connection between the patient's functional deficit and the resulting cost of hospitalization has been proved, and the thesis contains, among others, recommendations for the DRG payment system depending on the categories of patients in institutional care.
