Abstract

Analýza nákladové efektivity vybraných metod hodnocení hemostázy Cílem této diplomové práce bylo zhodnotit diagnostiku poruch hemostázy pomocí odlišných diagnostických metod se zaměřením na standardní koagulační testy a viskoelastické metody. Konvenční postupy byly zastoupené vyšetřením aktivovaného parciálního času, protrombinového času, trombinového času, antitrombinu III, fibrinogenu trombinového času, D Dimery a fibrinovými monomery. Viskoelastické testy zahrnovaly jeden test na vnější cestu koagulační aktivace a druhé test na vnitřní cestu srážení. Metody byly mezi sebou porovnávány u plánovaně přijatých pacientů z operačních sálů. Dílčími cíli bylo s pomocí vícekriteriálního rozhodování stanovit kritéria hodnocení jednotlivých alternativ a stanovit jejich váhy. Následně provést analýzu nákladové efektivity, vypočítat dobu návratnosti investice a udělat citlivostní analýzu. Z pohledu poskytovatele zdravotní péče je nákladově efektivnější variantou použití viskoelastických testů.

Cost-effectiveness analysis of selective methods of haemostatis evaluation The aim of this diploma thesiswas to evaluate the diagnosis of haemostasis defects by using different diagnostic methods. Conventional procedures were performed by examining activated partial tromboplastine time, prothrombin time, thrombin time, antitrombin III, fibrinogen, thrombin time, D dimers and fibrin monomers. Viscoelastic tests included one assay off external coagulation activation and one assay of internal coagulation activation. These two methods were compared between each other while accepting patients from operating rooms on purpose. Partial objectives were to set the criteria for evaluating individual alternatives and their weights. Consequently, perform a cost-efectiveness analysis, calculate the pay back time of both investments and do a sensitivity analysis. From the point of view of the healthcare provider, a more cost-effective option of using viscoelastic tests is a more cost-effective approach.
