Nestability kolenního kloubu při insuficienci předního zkříženého vazu a porovnání jejich následné terapie

Abstract

V práci se pojednává o problematice nestabilního kolenního kloubu zapříčiněné insuficiencí předního zkříženého vazu a způsobu následné léčebné terapie této instability. Obecná část práce je zaměřena na teoretické poznatky v oblasti poranění předního zkříženého vazu, především na mechaniku úrazů, jejich dopad na kolenní kloub a způsob léčby vzniklých nestabilit spojených s rupturou vazu. V této části práce jsou popsány vhodné rehabilitační metody s ohledem na to, jaký typ chirurgického zákroku byl proveden. Autorka se v práci zabývá konkrétními příklady léčebných postupů, které lze při insuficienci vazu aplikovat. Ve speciální části je posuzován status praesens dvaceti jedna probandů, kteří v období před dvěma až osmi lety podstoupili rekonstrukci předního zkříženého vazu. V závěrečném výstupu jsou zahrnuty subjektivní a objektivní faktory, které ovlivnily zdravotní stav kolenního kloubu a měly zásadní význam pro rekonvalescenci pacienta a jeho následné zařazení zpět do běžných denních a sportovních činností. Hlavním faktorem, který průběh rekonvalescence ovlivnil, byl přístup a zájem pacienta o rehabilitační péči. Získaná data byla zpracována a vyhodnocují vhodný léčebný postup po rekonstrukci vazu.

The paper deals with an unstable knee joint caused by an insufficient anterior cruciate ligament (ACL), and the methods of follow-up treatment therapy for such an instability. A general part of the paper is focused on a theoretical knowledge regarding anterior cruciate ligament injuries, especially on injury mechanisms, their impact on a knee joint, and on treatment methods for treating incurred instabilities caused by a ligament rupture. This part of the paper describes suitable rehabilitation methods with respect to a particular type of the surgery carried out. The paper's author follows concrete examples of treatment procedures applied for the insufficient ligament. The paper's main part evaluates a "status praesens" (present condition) of twenty one cases who underwent an anterior cruciate ligament reconstruction two to eight years ago. The final part contains both subjective and objective factors which influenced a knee joint health state, and had a fundamental importance for a patient's convalescence and his/her subsequent return to common daily and sport activities. The patient's interest and approach to the physiotherapeutical treatment were the main factors influencing a convalescent's progress. Collected data have been processed in order to assess a suitable treatment procedure after a ligament reconstruction.
