Porovnání nákladů na angiografii dolních končetin při využití magnetické rezonance a výpočetní tomografie

Abstract

Tatodiplomovápráce si klade za cílporovnat náklady na angiografii dolních končetin při využitímagnetické rezonancea výpočetní tomografie. Vúvodu jsme pomocí rešerše české i zahraniční literatury zmapovali současný stav problematiky využívání magnetické rezonance a výpočetní tomografie při angiografii dolních končetin zfinančníhoidiagnostického přínosu. Kezjištění skutečných nákladů na tato vyšetření a kanalýze rozdílů mezi reálnými částkami a ve financování daných výkonů pojišťovnou jsme využili metodu kalkulace neúplných (variabilních) nákladů. Výsledky této metody jsme porovnali svýsledkymetody AHP(analytickýhierarchický proces) multikriteriálního rozhodování. Ke stanovení vah jednotlivých kritériía jednotlivých alternativ jsme použili hodnocení expertní skupiny. Vrámci diplomové práce jsme také pomocí retrospektivní studie vymezili nejvíce indikované diagnózy kvyšetření MR a CT angiografie. Na základě zjištěných výsledků kalkulace neúplných nákladů a AHP nám jako lepší volba pro angiografii dolních končetin vyšla magnetická rezonance. Z indikovaných diagnóz se na prvníchmístech umístilystenózy ilických tepen, časté je i podezření na PAD.

This thesis aims to compare the costs of lower extremity angiography using magnetic resonance imaging and computed tomography. In the introduction, we are using a search Czech and foreign literature mapped the present state of the use of magnetic resonance imaging and computed tomographyangiography of the lower extremities, both financially and diagnostic contribution. To determine the real cost of the examination and analysis of the differences between real amounts of financing and the performance of the insurance company, we used the method of calculation incomplete (variable) costs. The results of this method were compared with the results of the methods AHP (Analytical Hierarchy Process) multi-criteria decision making. To determine the weights of individual criteria and the alternatives we used the evaluation of the expert group. The thesis we also use retrospective Limited possible diagnoses indicated for MRI and CT angiography. Based on the findings of the outcome of incomplete calculations of costs and AHP us as the better choice for lower extremity angiography, magnetic resonance emerged. From the indicated diagnoses were placed on top of iliac artery stenosis, frequently is suspected PAD.
