Fyzioterapie po traumatickém poranění páteře a míchy u pacienta s kvadruparézou

Abstract

Tato bakalářská práce se zabývá traumatickým poraněním páteře a míchy. Obecnáčást se zaměřuje na teoretické poznatky o anatomii, fyziologii, patofyziologii a traumatologii páteře amíchy. Dále tato část obsahuje též informace, které se týkajíklinických projevůpo poranění páteře amíchy, a dosavadní znalosti ojednotlivých diagnostických a terapeutických postupech. Speciální část se zaměřuje na možnosti léčebné rehabilitace spinálního pacienta. Součástí bakalářské práce je i konkrétní kazuistika pacienta, u něhož byla diagnostikovánakvadruparéza-plegie DKK a akroparéza HKK, která vznikla následkem traumatickéhoporanění páteře a míchy voblasti C7. Součástí kazuistiky jsou také informace o nemocném tj. vstupní data, anamnéza, indikace k rehabilitacia kineziologický rozbor, na základě kterého byl po důkladném seznámení se scelkovou pacientovou diagnózou a jeho zdravotním stavem následně vypracován krátkodobý fyzioterapeutický plán pro daného pacienta skvadruparézou. Na předcházející údaje navazuje dále protokol o průběhu jednotlivých fyzioterapeutických jednotek, které byly zacíleny na zlepšení celkového zdravotního stavu pacienta. Důraz byl taktéž kladen nejen na zmírnění jeho fyzických obtíží, ale i nausnadnění pacientovy sebeobsluhy a mobility. Vrámci zlepšenípacientovy mobility a sebeobsluhy jsme se soustředili mimo jiné i na nácvik samostatného přesunu pacienta zauta a do auta, které si právě ztohoto důvodu pořídil, ale pozornost bylahlavně věnována minimalizaci subjektivních obtíží. Nedílnou částí práce je také výstupní kineziologický rozbor a návrh dlouhodobého fyzioterapeutického plánu. Závěr ečná částpráce se pak věnuje objektivnímu zhodnocení výsledků terapie na základě porovnání vstupního a výstupního kineziologického rozboru a je zdezkonstatováno, zda došlo ke zlepšení subjektivních pocitů pacienta.

Thisbachelorthesis deals with traumaticinjury to thespine and spinal cord. The general section focuses on theoretical knowledge of anatomy, physiology, pathophysiology and traumatology of the spineand spinal cord. Furthermore, this section also contains information relating to the clinical manifestations of the spine and spinal cord injuries, and current knowledge about different diagnostic and therapeutic procedures. Thespecial section focuses on the possibilities of rehabilitation of a spinal patient. The bachelor thesis includes aspecific case report of a patient who was diagnosed with quadriparesis -paraplegia and acroparesis of upper extremities, which was the result of a traumatic injury to the spine and spinal cord at the C7 vertebra. The case report alsocontainsinformation about the patient, i. e. baseline characteristics, medical history, indications for rehabilitation and kinesiological analysis, which provided the basis for a subsequently developed short-term physiotherapy plan for the patient with quadriparesis after thorough acquaintance with the patient's diagnosis and overall medical condition. Thesedata are followed by a report on the courseof individual physiotherapy units, which were aimed at improvingoverall health of the patient. Emphasis was placed not only on alleviation of patient's physical ailments, but also on facilitation ofpatient'sself-care and mobility. In order to improve the patient's mobility and self-care, we focused, among other things, onpractice of a separate transfer of the patient from one car to another, that he just bought for this purpose. Nevertheless, our attention was mainly directed atminimizing the subjective complaintsof the patient. Final kinesiologicalanalysis andadesign of a long-term physiotherapy planrepresent an integral part of the thesis. The final part is devoted to objective evaluation of the therapeutic results by comparing the baseline and finalkinesiological analysis. Furthermore, the final section provides a notion, whether an improvement inpatient's subjective feelings was made.
