Fyzioterapie po frakturách distálního radia

Abstract

Předmětem této práce je zpracování problematiky fyzioterapie po frakturách distálního radia. Obecná část je věnována teoretickým poznatkům týkajících se tohoto tématu. V této části je popsána anatomie a kineziologie zápěstí a loketního kloubu, historie léčby fraktur distálního radia, klasifikace zlomenin, mechanismus úrazu, léčba a také přidružená poranění. Hlavní, tedy speciální část, je zpracována formou kazuistik dvou pacientek po osteosyntéze, u kterých se srovnává úspěšnost fyzioterapie v závislosti na časové délce fixace končetiny. Další částí je metodologie práce, kde jsou popsány veškeré vyšetřovací a terapeutické metody a postupy, které byly použity k vypracování části speciální. Při tvorbě vstupního a výstupního kineziologického rozboru se vychází z popsaných vyšetřovacích metod. K sestavení a následné realizaci cvičebních jednotek jsou použity popsané terapeutické metody. Součástí této kapitoly je také sběr dat, který zahrnuje popis pracovišť, ve kterých byla speciální část zpracována. Speciální část se zabývá kazuistikami dvou pacientek po osteosyntéze distálního radia, přičemž první pacientka je po operaci bez sádrové fixace a druhá se sádrovou fixací. Součástí kazuistik je vstupní kineziologický rozbor, na základě kterého je sepsán závěr vyšetření a navržen krátkodobý a dlouhodobý rehabilitační plán. Ve speciální části jsou také popsány jednotlivé terapeutické jednotky každé z pacientek včetně cíle, průběhu a také efektu terapie. V kapitole výsledky jsou vypracovány výstupní kineziologické rozbory, ve kterých je graficky znázorněno zlepšení či zhoršení každé z pacientek. Tato část je zpracována na základě samostatné práce s pacientkami na příslušných rehabilitačních oddělení. V kapitole závěr a diskuze je vyhodnocena úspěšnost léčby pacientek a vzájemné srovnání výsledků terapie.

The objective of this bachelor thesis is to process the issue of physiotherapy in patients with fractures of the distal radius. The general section deals with theoretical knowledge on this subject. This section describes the anatomy and kinesiology of the wrist and elbow joint, the history of treatment of distal radius fractures, classification of fractures, mechanism of injury, treatment, and associated injuries. The main, special, section provides case reports of two patients who underwent an osteosynthesis, and in whom the efficacy of physiotherapy is compared depending on the duration of fixation of a limb. The next section of the thesis, methodology, describes all examination and therapeutic methods and procedures that were used in the special section. The design of the baseline and final kinesiological analysis is based on the described methods of examination. The described therapeutic methods were used for the preparation and subsequent implementation of the training units. This chapter also provides a collection of data, including a description of the workplaces, in which the special section was performed. The special section deals with case reports of two patients who underwent an osteosynthesis of the distal radius. The first patient underwent a surgery without plaster fixation and the other one with a plaster fixation. The case reports include a baseline kinesiological analysis, based on which a short-term and long-term rehabilitation plan was proposed. The special section also describes the different therapeutic units of each of the patients, including the objectives, course and effects of the treatment. The results section provides the final kinesiological analysis, in which an improvement or deterioration of any of the patients is graphically demonstrated. This section is compiled on the basis of individual work with the patients at the particular rehabilitation departments. The conclusion and discussion chapters evaluate the treatment efficacy in the patients and compare the results of the therapy
