Komplexní péče o dítě s kvadruparetickou formou dětské mozkové obrny se zaměřením na fyzioterapii

Abstract

Obecná část se zaměřuje na teoretické poznatky o klasifikaci forem DMO, etiologii, patofyziologii, prevenci vzniku, diagnostice a přidružených chorobách. Dále jsou vteoretické části rozebrány léčebné postupy ortopedické, neurochirurgické, farmakologické, otorinolaryngologické a foniatrické. Popsána je také intervence speciální pedagogiky, sociální rehabilitace, logopedie, ergoterapie, protetiky, hipoterapie, canisterapie a komplexní lázeňské léčby. Podrobněji jsou pak rozebrány jednotlivé speciální prostředky léčebné rehabilitace. Ve speciální části práce je vypracována kazuistika desetileté pacientky se spastickou kvadruparetickou formou DMO, spozitivní perinatální anamnézou. Konkrétní kazuistika obsahuje vstupní data pacientky, detailní anamnézu, podrobnosti o předchozí rehabilitaci a výpis ze zdravotní dokumentace. Vypracován je kineziologický rozbor, na jehož základě je navrhnut krátkodobý fyzioterapeutický plán. Vnávaznosti na tyto údaje je uveden protokol sprůběhem jednotlivých fyzioterapeutických jednotek, které byly prováděny za účelem zlepšení nebo alespoň udržení stávajícího stavu pacientky. Důraz byl kladen na zlepšení sebeobslužných úkonů a částečné dopomoci při denní péči rodiny a ošetřovatelů. Pozornost byla věnována také prevenci vzniku sekundárních komplikací. Na závěr byl vypracován výstupní kineziologický rozbor a návrh dlouhodobého fyzioterapeutického plánu. Vzávěru je objektivně zhodnocen výsledek terapie na základě porovnání vstupního a výstupního kineziologického rozboru a také je zahrnut subjektivní pocit pacienta a jeho rodiny.

The general part focuses on theoretical the reseach findings regarding the forms of classification of DMO, etiology, pathophysiology, prevention, diagnosis, and allied illnesses. The theoretical part of the work continues by the analysis of orthopedic, neurosurgical, pharmacological, ENT treatment procedures, and phoniatrics therapy. There is also described the intervention of special education, social rehabilitation , speech therapy , occupational therapy , orthotics , hippotherapy , canistherapy, and full spa treatments. There are more details of the various special rehabilitation resources discussedas well. The case history of a 10-year-old patient with the quadruparetic form of spastic cerebral palsy and positive perinatal medical history are describedin the special part of this work. Specific case report contains the patient input data, detailed medical history, details of the previous rehabilitation treatments, and an extract from the medical records. There is akinesiology analysis developed on the basis of which a short-term physiotherapy planis designed. Following the said dataa protocolis presentedwith the course of individual physiotherapy unitsthatwere implemented in order to improve, or at least maintain, the current status of the patient. Emphasis was placedon improving self-care tasks, and partial support in dailycare provided bythe caregivers and family members. Attention was also given to the prevention of secondary complications. There was anoutput kinesiology analysis made and the long-term physiotherapy plan drafted asthe result. In the conclusion, this bachelor thesis includes the objective evaluation of the therapy effect by comparing the input and output kinesiology analysis an d thesubjective feeling of the patient and her family.
