Možnosti léčby a fyzioterapie po poranění šlach flexorů prstů

Abstract

Předmětem bakalářské práce je zpracování problematiky zabývající se fyzioterapeutickým postupem a využitímněkterých terapeutických metod u pacientů po poranění šlach flexorů prstů. Tato bakalářská práce je zpracována formou kazuistiky upacientky po poranění flexorových šlach a nervů voblasti zápěstí. Tato diagnóza byla vybrána zdůvodu malého povědomí laické veřejnosti, ale i fyzioterapeutů. Jedná se o specifické poranění, které vyžaduje pro dosažení optimálníchvýsledků odbornou a specializovanou péči. Bakalářská práce je rozdělena na část teoretickou, metodologickou a speciální. Vteoretické části je stručně popsána anatomie horní končetiny sdůrazem na anatomii ruky. Další oddílteoretické části je věnovánpopisu postižení periferního nervu a příznakůperiferních paréz ruky. Konečnápasáž teoretické části zahrnuje kliniku poranění sdůrazem na operační přístupy, suturu šlach, cév a nervů a na pooperační léčbu. Metodologie práce popisuje použité vyšetřovací a terapeutické metody vhodné pro vybranou diagnózu. Speciální část je tvořena písemným souhrnem získaných dat ze vstupního vyšetření, na základě kterého je navržen krátkodobý a dlouhodobý rehabilitační plán. Součástí krátkodobého rehabilitačního plánu je popis deseti terapeutických jednotek. Závěrem speciální části je výstupní kineziologickévyšetření, které je podkladem pro kapitolu zhodnocení proběhlé terapie a závěr. Vdiskuzi zohledňuji operační přístup vČR a vzahraničí a popisuji vývoj rehabilitace po poranění šlach flexorů prstů. Dále zhodnocuji proběhlou terapii a popisuji některá svá pozorováníběhem zpracování práce. Vzávěru práce jsoukriticky zhodnoceny dosažené výsledky.

This work is madeas a case study inapatient after an injury of the flexor tendons and nerves in the wrist. This diagnosis was chosen because of its low awareness among the generalpublic, but alsoamongphysiotherapists. Itis a specific injury that requires professional and specialized care to obtain optimal results. Thisbachelor thesis is divided into atheoretical, methodological and specialpart. The theoretical part briefly describes the anatomy of the upper limb with an emphasis on the anatomy of the hand. Another section of the theoretical partis devoted to a description of peripheral nerve impairment and symptoms of peripheral paresisof thehand. The final sectionof the theoretical part coversthe clinical approach tothe injurywith emphasison surgical approaches, suturingoftendons, blood vessels and nerves and postoperative treatment. The methodologicalpart describes examinationand therapeuticapproacheswhich were used in thisthesis and which aresuitablefor the chosen diagnosis. The special part is composedof a written summary of the data obtainedthroughinitialmedical examination. A short-and long-term rehabilitation planis proposed based on this examination. The short-term rehabilitation plan includesa descriptionof ten therapeutic units. The conclusion of the special part isthe finalkinesiology examination, whichserves asa basis for the chapter evaluating the elapsed therapy and as a basis for conclusion. The discussionconsiders surgicalapproach in the Czech Republic and abroad, and describes the developmentsof rehabilitation after the finger flexor tendon injury. Further, it evaluates the elapsed therapy anddescribes some of the author's observationsmade during the processing of the thesis. In the conclusion of the thesis, the obtained results are critically evaluated.
