Termická poranění v přednemocniční neodkladné péči se zaměřením na poranění elektrickým proudem

Abstract

V mé práci se věnuji popáleninám v přednemocniční péče následkem působení vysoké teploty a také elektrotraumatům. Tyto úrazy nejsou moc časté a tak si myslím, že je nemají zdravotnicí příliš zažité. To je jeden z hlavních důvodů, proč jsem si toto téma zvolila pro svou práci. V teoretické části shrnuji poznatky jak z oblasti anatomie a fyziologie kůže, tak i patofyziologických procesů. V dalším uvádím dělení popálenin, hodnocení závažnosti popáleninového úrazu a možné komplikace. Součástí teorie j V další části teorie se věnuji laické první pomoci a navazující přednemocniční péči. Zejména se zaměřuji na péči poskytovanou posádkou ZZS bez lékaře, kde podrobně popisuji celkové vyšetření a zajištění pacienta. V poslední části se věnuji elektrickému proudu, jeho účinkům na organismus a faktorům, které ovlivňují závažnost poranění elektrickým proudem. Píši také o lokálním poškození při zásahu elektrickým proudem. Zde popisuji jak laickou, tak profesionální první pomoc a možné komplikace elektrotraumatu. Zvláštní pozornost je také věnována kardiopulmonální resuscitaci posádkami ZZS. V praktické části se věnuji intervencím, které byly provedeny posádkami ZZS u pacientů s popáleninami a elektrotraumaty. Podle vytvořeného protokolu sleduji, zda byly požadované úkony u pacientů provedeny či nikoliv. Cílem bakalářské práce je vytvořit algoritmus zdravotnického záchranáře při péči o popáleného pacienta se specifiky úrazu elektrickým proudem.

My thesis deals with burns in pre - hospital care resulting from the effect of high temperature and also electro trauma. These injuries are quite rare and therefore I assume that medical staff might not be properly experienced here. It is one of the main reasons why I have chosen this topic for my work. The theoretical part summarizes knowledge both from the field of the anatomy and physiology of skin and pathophysiological processes. Further, I continue with the division of burns and the evaluation of the seriousness of the injury due to burning. I also mention possible complications. In another part of the theory I deal with lay first aid and with follow - up pre - hospital care. I mainly focus on care provided by the Emergency Medical Service crew without a doctor where I discribe in detail the general examination and securing of the patient. The final part deals with electric current and its effects, factors that influence the seriousness of injuries caused by the electric current. I also mention local damage after electric current injury. Here I also describe both lay and professional first aid as well as possible complications of electro trauma. Special attention is also paid to cardiopulmonary resuscitation provided by the Emergency Medical Service crew. The practical part deals with interventions that were made by EMC crews in patients who suffered burns and electro trauma. With the worked out protocol I monitor whether certain operations were made in patients or not. The aim of the thesis is to create an algorithm of paramedic taking care of burned patient with the specific injury of an electric current.
